| Objective: Nowadays,most of the studies,focus on MTAD and sodium hypochlorite root canal irrigations,are limited to original research,it is pointed out that there are three common characteristics of removing smear layer,sterilization and dissolving pulp in two irrigations.In this paper,we use the systematic review to explore whether MTAD can replace sodium hypochlorite in sterilization and removing the smear layer and to hypothesis test their differences among ability of removal smear layer,bactericidal ability(enterococcus faecalis)and efficiency of sterilization.Method: The electronic search of the Pubmed,Embase,Ovid MEDLINE(R)In-Process & Other Non-Indexed Citations,Ovid MEDLINE(R)Daily and Ovid MEDLINE(R)(via.ovid),Cochranelibrary,CNKI,WANFANGDATA,Sino Med Database was performed,and manual retrieval covered books of library of Shanxi Medical University and stomatology hospital and a variety of oral medical journals and related conference literature.According to the PICOS principle,the inclusion and exclusion criteria,the data extraction and bias risk assessment were managed.SMD and OR were selected as indicators of efficiency of sterilization and ability of removal of smear layer by means of META analysis though revman5.3.When 50% < I2 < 75%,analyzed the experimental design,apparatus and place.Selected random effect model and accepted the results prudently while heterogeneity could not be find.when I2 > 75% and heterogeneity could not be find,systematic review was used.Use the Greadpro3.6.1 to assess the quality of evidence and the AMSTAR-list to assess the quality of method.Result: At last,we totally searched out 220 related literatures.In the first selected we exclude 157 literatures,the second time we exclude 6 of them.Finally,15 studies were brought into.There were a total of 483 cases of samples.95% CI of ability of removal of smear layer,included 4 literatures,total of 60 cases,the meta analysis show that:95%CI:-1.58 [-2.35,-0.82],Z=4.07,P < 0.0001,the results were statistically significant.Bactericidal ability(enterococcus faecalis),included 3 literatures,total of 103 cases of samples,the meta anaylsis show that 95%CI of SMD of bactericidal ability(enterococcus faecalis)was 0.26[-0.14,0.65],P=0.2,the results were not statistically significant.As the results of third outcome Diameter of bacteriostatic ring heterogeneity,meta analysis was given up and systematic review was used.we included 4 literatures,100 cases of samples,there were results:95%CI of Tay group:3.89 [2.62,5.17],the results were statistically significant;95%CI of Li Xinjun group:-0.43 [-1.15,0.30],the results were no difference between them;95%CI of Yu Xin group:16.17 [11.10,21.23],the results were statistically significant;95%CI of Torabinejad:0.36 [-0.63,1.35],the results were no statistically significant.;at last outcomes---sterilization efficiency,included total of 220 of samples,the meta anaylsis show that OR>1,Z=4.72,95%CI:14.80 [4.84,45.29].,the results were statistically significant,MTAD is superior to sodium hypochlorite.Conclusion: MTAD could be considered superior to sodium hypochlorite in sterilization and removing the smear layer through META analysis.In the bactericidal ability there is no statistically significant.But the rest of outcomes could not conclude a clear conclusions. |
