Objective: The aim was to evaluate the value of routine MRI feathers and DWI date in predicting the molecular pathological typing of glioma noninvasively.Methods: The routine MRI and DWI date with pathologically proved gliomas was retrospectively reviewed.The clinical date(sex?age),routine MRI date(location?number?midline structure?homogeneity?degree of cyst or necrosis?hemorrhage?border?edema?enhancement?diameter)and ADC date(mADC?rmADC?eADC?reADC)were evaluated.According to the World Health Organization classification of tumors of the Central Nervous System grading in gliomas,the lesions were divided into low grade(including WHO grade?)(39 patients)and high grade(including WHO grade?and WHO grade?)(66 patients).IDH1 gene status was determined by IDH1R132 H antibody immunohistochemistry and gene sequencing.In low and high grade glioma,27 and 21 patients showed IDH1 mutation.Using the same WHO grading,the clinical date and imaging parameters of patients with IDH1 mutation and wild were compared to determine useful metrics for differentiating the mutation.Results:1.clinical data:High grade gliomas with IDH1 mutation were apt to younger patients.2.routine MRI data:IDH1 mutant tumors were prone to frontal location;High grade gliomas can be multiple and infiltration;Low grade gliomas patients with IDH1 mutation can be detect by midline structure and diameter?degree of cyst or necrosis)IDH1 mutant patients were presented as different degree of edema and enhancement from wild patients.3.DWI data:The mADC and rmADC values had statistical significance between low and high grade gliomas patients with IDH1 mutation and wild.In low grade,the areas under the curve(AUC)for mADC and rmADC were 0.819?0.823,respectively,and the cutoff value were 1.265×10-3mm2/s?0.283;In high grade,the AUC for mADC and rmADC were 0.872?0.869,the cutoff value were 0.094×10-3mm2/s?0.389.Conclusion:1.In clinical date,age can helpful for detect IDH1 gene status in low grade gliomas;2.Some routine MRI parameter can use to evaluate IDH1 gene status noninvasively in both low and grade gliomas;3.mADC and rmADC has high reference value to judgment IDH1 gene status in gliomas.
