The Development And Application Of Health Literacy Scale For Perinatal Pregnant Women

Objective : To develop "the health literacy scale for perinatal pregnant women",and to explore the health literacy of perinatal pregnant women and influence factors in one district of Chongqing,so as to provide the basis for the further development of the health literacy measurement instruments for perinatal pregnant women and improve the health literacy intervention of perinatal pregnant women and improve their health literacy.Methods : Used the Nutbeam D professor’s functional,communicational,critical health literacy level theory,combined with a large number of domestic and foreign literatures,the discussions of the group,consulted and investigated 18 experts.Formed 58 items of pregnant women in the perinatal period health literacy original scale.On 2016 3-5,a total of 477 pregnant women were investigated of one General Hospital and two Maternal and Child Health Hospitals of one country,the 7 methods were used to select the items of the original scale,eventually formed "the health literacy scale for perinatal pregnant women",which contained 51 items and feasibility,reliability,validity were evaluated.On 2016 9-11,a total of 523 perinatal pregnant women were investigated,and the database was established by Epidata3.1,and SPSS17.0 was used to analyze the perinatal health literacy and its influencing factors.Results:Analysis of expert consultation results showed that the positive coefficient was 90%,and the average authority coefficient was up to 0.87.Through the selection of items and reliability and validity test,we had completed the preparation of "the health literacy scale for perinatal pregnant women",which contained 51 items.The scale was divided into 11 dimensions and 3 areas: functional health literacy,communicational health literacy and critical health literacy.The evaluation showed that the scale recovery rate reached 95.4%,the completion rate of 100%,the completion time was within 20 minutes;the total scale and the three areas of Cronbach’s alpha coefficient were 0.909,0.879,0.848,0.843 and split half reliability coefficient were 0.885,0.802,0.773,0.823;three areas with total scale of correlation coefficient between 0.690-0.784.The influence factors analysis showed that the structure of the scale was consistent with the theory.Current research showed that the total,functional,communicational,critical and overall health literacy of perinatal pregnant women were 35.8%,16.1%,7.1% and 14.0%;the township,junior high school and below,junior high school and below of her husband,family income below 4000 and below,not check-up on time,not willing to accept prenatal education,introverted,not good relationship with in-law parents and spouses of the perinatal health literacy were low(all P<0.05).Multivariate logistic regression analysis showed that the education of perinatal pregnant women,checked-up on time or not,accepted prenatal education or not were the influencing factors of functional health literacy;accepted prenatal education or not,the relationship with in-law parents were the influencing factors of communication and total health literacy;family income,the relationship with in-law parents were the influencing factors of critical health literacy.Conclusion: "The health literacy scale for perinatal pregnant women" has good feasibility,reliability and validity,and can be used as a tool to evaluate the health literacy of perinatal pregnant women.The health literacy level of perinatal pregnant women is influenced by many factors,taking corresponding intervention measures against these factors are the key to the next step.
