Therapeutic Analysis Of Laparoscopic Sleeve Gastrectomy For Obesity Combined With Type 2 Diabetes Mellitus

Objective:To explore t the safety and efficacy of laparoscopic sleeve gastrectomy for the treatment of obesity complicated with type 2 diabetes mellitus.Methods:12 obesity patients combined with type 2 diabetes mellitus conducted LSG in the Affiliated Dayi Hospital of Shanxi Medical University from April 2013 to May 2015 were selected as the subjects.All patients were followed up for 12 months after operation,the changes of related index like weight,body mass index(BMI),excess weight loss rate(EWL%),fasting plasma glucose(FPG),fasting insulin(FINS),fasting C-peptide(FCP),hemoglobin A1C(HbA1C),homeostatic model assessment of insulin resistance(HOMA-IR),triglyceride(TG),total cholesterol(TC),low-density lipoprotein(LD L),high-density lipoprotein(HDL),uric acid,systolic and diastolic blood pressure of patients before and after operation were compared.Results:12 patients were successfully completed surgery,no conversion to open surgery during operation,no intraoperative bleeding,and no serious complications such as gastric leakage,gastric bleeding and gastroesophageal reflux were found after operation.The follow up time was 1、3、6 and 12 months,respectively,the body weight,BMI,FPG of patients at each time after operation was lower than that before operation(P<0.05);EWL% showed an upward trend(P<0.05).Other metabolic indicators,such as FINS,FCP,HbA1 C,HOMA-IR,TG,TC,LDL,HDL,uric acid,systolic blood pressure and diastolic blood pressure were significantly improved compared with preoperative;Some patients were followed up to 12 months after operation,and found that fasting blood glucose and body weight decreased slowly,and gradually entered the plateau stage.Conclusion:LSG weight loss hypoglycemic effect is exact,safe control and Short-term efficacy is satisfactory;due to the small number of cases and the short follow-up,the long-term efficacy remains to be further studied.
