| Objective: To study the risk factors and clinical features of premature ovarian insufficiency(POI),improve the understanding of POI,and provide a theoretical basis for the early diagnosis and prevention of these patients.Methods: The clinical data of 63 POI patients diagnosed by the gynecological clinic of the First Hospital of Shanxi Medical University from December 2014 to July 2016 were collected,which included the relater risk factors,such as: chromosome karyotype;hypothyroidism,Hashimoto thyroiditis,hyperparathyroidism,systemic lupus erytematosus,adrenal insufficiency and other autoimmune diseases;pelvic surgery;mumps,tuberculosis,measles,varicella and other infections;the history of smoking or secondhand smoking;A-type behavior questionnaire;and clinical data,such as: age;occupation;Menopausal menstrual period or menstrual period,history of pregnancy and abortion.Results: 1.In the 63 cases of POI,the youngest was 17 years old and the oldest was 40 years old.The mean age was 26.59 ± 6.29 years.The patients younger than 30 years old were 49(77.78%).The patients older than 30 years old were 14(22.22%).2.The etiology of 63 cases of POI showed that the chromosomal abnormalities 9 cases(14.29%),autoimmune dysfunction 3 cases(4.76%),iatrogenic factors 2 cases(3.17%),infectious factors 11 cases(17.46%),environmental factors 39 cases(61.9%),A-type behavior pattern 38 cases(60.32%)and unknown causes 7 cases(11.11%).3.In 63 POI patients: primary amenorrhea 10 cases(15.87%),oligomenorrhea 15 cases(23.81%),secondary amenorrhea 38 cases(60.32%).In the 38 cases,35 cases(55.56%)showed menstrual abnormalities before amenorrhoea,3 cases(4.76%)showed regular menstrual before amenorrhoea.In the 10 cases of primary amenorrhea,chromosome abnormal 6(60%),adverse environmental factors exposured 3(30%),A-type behavior pattern in 3 patients(30%).In the 15 cases of oligomenorrhea,A chromosome was abnormal in 1 case(6.67%),adverse environmental factors exposured 12(80%),A-type behavior pattern 10(66.67%).38 cases of secondary amenorrhea,abnormal chromosomes in 2 cases(5.26%),adverse environmental factors exposured 24(63.16%),A-type behavior pattern 25 patients(65.79%).4.Of the 63 patients with POI,14(22.22%)have previously had a pregnancy,12(85.71%)of whom had abortions.Conclusion: 1.This study showed the average age of 63 patients were 26.59 ± 6.29 years old.2.The etiology of POI is complex,clinical multi-factor investigation should be tested for patients,so that we can take the corresponding treatment.3.Chromosomal abnormalities have a significant influence in primary amenorrhea.Adverse environmental factors and personal life behavior have a significant influence in oligomenorrhea and secondary amenorrhea.4.Most of the POI patients showed menstrual abnormalities.We should pay attention to assess ovarian function and fertility preservation for these people. |
