Meta-analysis Of Effectiveness Of Cardiac Resynchronization Therapy In Patients With Mild Heart Failure

Objective: To evaluate the effectiveness of cardiac resynchronization therapy(CRT)on patients with mild heart failure(HF)by Meta-analysis.Method: The literature search was all most conducted in electronic databases(Medline/Pubmed,Foreign language biomedical database in full,CNKI,wanfang database,OVID,Embase,Google Scholar)from their inception through 2009 to 2014,find the RCTs reporting the effects of CRT on clinical outcomes and echocardiographic results in patients with mild HF.Data regarding infection,a11-cause mortality,heart failure events,left ventricular ejection fraction(LVEF),LV endsystolic diameter(LVESD),LV end-diastolic diameter(LVEDD),LV end-systolic volume(LVESV),LV end-diastolic volume(LVEDV),were meta-analyzed and extracted by using Review Manager5.3 software.Result:Seven randomized contro11 ed studies were found according to the criteria,mild heart failure including 4317 patients with New York heart function classification I,II(NYHA I/II).The result did not shows that infection different between the CRT and the control group,RR=1.46 [ 95%CI(0.86,2.49);P=0.16 ];After CRT,the incident of heart failure events was reduced significantly;RR =0.72 [ 95%CI(0.63,0.82);P<0.00001 ];a11-cause mortality RR =0.87 [ 95%CI(0.72,1.05);P =0.14],there was no obvious statistical significance difference;the LVEF ’ S MD was 5.35 [ 95%CI(-0.05,10.75);P=0.05]there was obvious statistical significance difference;the LVESV ’ S MD was-39.16 [ 95 % CI(-41.33,-36.99);P < 0.00001 ];there wasobvious statistical significance difference;the LVESD’S MD was-5.75 [ 95%CI(-8.37,-3.12);P < 0.0001 ];there was obvious statistical significance difference;the LVEDD ’ S MD was-4.50 [ 95%CI(-4.72,-4.27);P <0.00001 ];there was obvious statistical significance difference;the LVEDV’S MD was-37.47 [ 95%CI(-39.68,-35.26);P<0.00001 ]here was obvious statistical significance difference;there was obvious statistical significance difference;the LVESVi’S MD was-20.91 [ 95%CI(-25.33,-16.49);P<0.00001 ]here was obvious statistical significance difference.Conclusion:For the patients who are mild heart failure,CRT can reduce the incidence of heart failure events,and reverse the left ventricular re-construction obviously.We didn ’ t found that CRT can increase infection and reduce a11-cause mortality obviously.
