Objective:Comparing the difference between cardiac resynchronization therapy pacemaker(CRT-P)alone and combined defibrillator(CRT-D)on the prognosis of patients with heart failure,and then provide certain guiding significance for clinical work.Methods:The Pub Med,Embase,Cochrane Library,How Net,Wanfang,and Weipu databases are searched by computer.The search time is from the establishment of the database to December 2019.The literature is screened according to the pre-set inclusion and exclusion criteria,and the quality evaluation is performed according to the pre-designed The table extracts literature and related data,and uses the Rev Man 5.3 software provided by the Cochrane system to meta-analyze the related data.Results:A total of 27 articles were included(n=17393).The analysis results showed that the all-cause mortality in the CRT-D group was lower than that in the CRT-P group(OR=0.67,95%CI: 0.58-0.78,p<0.00001).The sudden death rate was also lower than that of the CRT-P group(OR=0.32,95%CI: 0.24-0.43,p<0.00001),while the heart failure mortality rate of the CRT-D group was not lower than that of the CRT-P group(OR=0.72,95%CI: 0.50-1.03,p=0.07).Conclusion:In terms of all-cause mortality and sudden cardiac death,the prognosis of the CRT-D is better than that of the CRT-P,but the CRT-D may not have a significant advantage over the CRT-P in terms of heart failure mortality.
