Study In Clinical Characteristic And Prognostic In Primary Intestinal Non-hodgkin’s Lymphoma

Objective: To study the clinicalcharacteristic,imagingand endoscopy,pathology,therapeutic methods and overall survival(OS)in patients with primary intestinal Non-Hodgkin’s lymphoma(PI-NHL).Methods: 68 cases of PI-NHL hospitalized in Zhangzhou Affiliated Hospital of Fujian Medical University from January 2001 to November 2015 were enrolled to this study.The correlation between Clinical characteristics,pathological type,therapeutic method,overall survival and prognosis was analyzed using χ2,Kaplan-Meier,Log-rank,Cox and ROC by SPSS 22.0.P<0.05 was considered significant.Results:In 68 cases,agewas range 3-88 years old(media was 57y),males 49 and females 19,with ratio2.58:1.There were 41 cases with abdominal pain,13 cases with abdominal distention,12 cases with diarrhea,5 cases with fever,5 cases with abdominal mass,5 cases with physically nauseated.29 cases(42.6%)had intestinal obstruction or enterobrosis.8 cases involve in duodenum,2 cases in conjunction of gastrointestinal anastomosis,5 cases in jejunum,19 cases in ileum,20 cases in ileocecum,18 cases in colon,9 cases in rectum.OS was 67.51%,54.61% and 54 % in 1,3 and 5yearrespectively.Tumors were size range 1 ~ 25 cm,36(52.9%)cases were at < 7.5 cm,32(47.1%)cases were at ≥7.5 cm,with 36.74 months and 73.85 months in OS,p=0.048.61(89.7%)cases were B cell lymphoma,7(10.3%)cases were T cell lymphoma and with 63.64 month and 38.43 month in OS,P =0.001.20 cases(29.4 %)were at Ann Arbor stagingI-II,11(16.2 %)cases at III,34(50.0 %)cases at IV.OS was 65.18 months,58.40 months,49.12 months in stage I-II、III、IV respectively,P=0.039.49 cases were treated with chemotherapy and 19 cases without chemotherapy,with 76.93 month and34.56 month in OS,P=0.001.8 cases with LYM<0.6x109/L(14.8%),46 cases(85.2%)≧0.6x109/L,with 15.09 month and 42.36 month in OS,P=0.039.There were no correlations between OS and age,gender,WBC,HGB,LDH,ESR,EB virus infection,Lugano staging,number of intestinal involve,lymph nodes number,B symptom,bone marrow infiltration.Kaplan-Meier multivariate analysis showed that LYM<0.6x109/L,Ann Arbor staging,chemotherapy were independent prognostic factors,and P<0.05.Conclusion:PI-NHL has nonspecific clinical characteristics.single factor analysis showed that LYM<0.6×109/L,T type lymphoma,Ann Arbor staging III,IV,bulk tumor,without chemotherapy were poor prognostic factor.COX multivariate analysis showed LYM<0.6×109/L,Ann Arbor staging,chemotherapy were independent factor.
