BackgroundThe pathogenesis of coronary artery heart disease(CHD)is extremely complex,and its core is atherosclerosis(AS)。At present,many studies have explored the relationship between inflammatory response and AS,and it has been shown that inflammatory response may be involved in the core mechanism of atherosclerosis.The incidence of atherosclerosis,especially coronary heart disease,is associated with a high level of inflammatory cytokines in the blood circulation,including monocyte chemoattractant activation factor(MCP-1),tumor necrosis factor(TNF-α),interleukin(IL)。Therefore,this study was aimed to explore the change of serum MCP-1、TNF-α and IL-6 in patients with CHD and its correlation with severity of coronary artery stenosis,in order to further explore mechanisms of CHD and to find novel therapeutic strategies.Aims1.To observe the difference between the levels of serum MCP-1,TNF-α and IL-6 in coronary heart disease group,atherosclerosis group and normal control group.2.To analyze the correlation between serum MCP-1,TNF-α and IL-6 levels and the degree of coronary artery disease in patients with coronary heart disease.3.To investigate whether serum MCP-1,TNF-α and IL-6 levels are independent risk factors for coronary heart disease.Methods1.A total of 112 patients with coronary heart disease diagnosed in the Department of Cardiology,General Hospital of Tianjin Medical University from March 2016 to October 2016 were admitted as subjects of experimental group of the study.A total of 69 patients with coronary atherosclerosis were diagnosed as AS group and 24 cases were diagnosed as normal coronary artery as normal blood vessel group.Patients with coronary heart disease were divided into three groups according to the main coronary arteries involved : 59 patients in the single vessel group,54 patients in the double vessel group,and 68 patients in the three vessels.According to the Gensini score,the patients were divided into three groups: mild stenosis group: score of 0 <Gensini ≤ 18 in 92 cases;moderate stenosis score of 18 <Gensini ≤ 41 in 49 cases;severe stenosis score Gensini > 41 points in 40 cases.2.All fasting subjects’ venous blood were taken in the morning 6: 00-7: 00,professionals in Tianjin Medical University General Hospital examination department in the day to determine liver and kidney function,fasting blood glucose,blood lipids four,myocardial enzymes,BNP and other indicators.The levels of IL-6,MCP-1 and TNF-α in serum were measured by liquid-phase chip analysis system(Luminex200 flow fluorescence detector: Luminex,USA).3.Statistical analysis was performed with SPSS 17.0 statistics software.Measurement data were presented as x ± s,and categorical variables were expressed as frequencies or percentages.All data were analyzed by the Kolmogorov-Smirnov and Leven.Multivariate metering data were compared using the variance analysis,and the count data were compared using chi-square test.Independent risk factors were analyzed using logistic regression analysis.All data were analyzed by SPSS17.0,with P <0.05 for the difference was statistically significant.Results:1.There was no significant difference in age,sex,smoking index,body mass index,hypertension,cholesterol,low density lipoprotein,triglyceride,alanine aminotransferase,creatinine and creatine kinase in coronary heart disease group,AS group and normal coronary artery group(P> 0.05).However,with the severity of coronary artery stenosis,patients with diabetes history(P <0.01),high-density lipoprotein(HDL-C)lower(P <0.05)increased,with statistical differences.There was no significant difference in the levels of serum IL-6 between the three groups(P> 0.05).There were significant differences in the levels of MCP-1 and TNF-α between the three groups(P <0.01).2.Patients were divided into single-vessel disease group,double-vessel disease group and three-vessel disease group according to the main coronary arteries involved in coronary heart disease.The results showed that there was no significant difference in IL-6 level between different stenosis patients(P> 0.05).There were significant differences in the levels of MCP-1 and TNF-α(P <0.01).And the more the number of blood vessels involved in the lesion,the more obvious the level of serum MCP-1 and TNF-α increased.3.According to the Gensini score,the scores were divided into three groups: mild stenosis group,moderate stenosis group and severe stenosis group.The levels of IL-6,MCP-1 and TNF-α in patients with different degree of coronary artery stenosis were significantly different(P <0.05,P <0.01,P <0.01).And the level of serum IL-6,MCP-1 and TNF-α increased with the severity of coronary stenosis.4.The results of univariate logistic regression and multivariate logistic regression analysis showed that MCP-1 and TNF-α were independent risk factors of coronary heart disease(P <0.01).IL-6 could not be considered as an independent risk factor for coronary heart disease(P> 0.05).
