[Objective]:To investigate the clinical effect of transpedicular fixation with or without injured vertebra in the treatment of thoracolumbar fracture.[Methods]:A retrospective analysis of 94 patients with thoracolumbar fracture of thoracic lumbar vertebra was collected.Among them,49 cases were treated as observation group and 45 patients were treated as control group.The operation time of the two groups was observed,intraoperative blood flow,drainage and drainage time were observed.After follow-up for 18 months,clinical indicators such as preoperative,one-week and post-operative follow-up were observed,and the rate of Cobb,internal fixation failure rate,VAS and ODI scores were observed.[Results]:Observation group and control group,the average age of the two groups of patients,gender ratio,operation time,intraoperative blood loss,flow and drainage tube indwelling time,internal fixation failure rate,VAS and ODI score has no obvious statistical significance(P>0.05).There was no significant statistical significance(P>0.05)in the anterior border of the vertebral body and the Cobb Angle in the first two groups.There was no statistically significant difference between the anterior margin of the vertebral body of the two groups and the Cobb Angle of the two groups after the postoperative period(P>0.0.05).Before and after surgery,both groups of patients were corrected by comparison of the anterior border of the vertebral body and the Cobb Angle,which was statistically significant(P<0.05).The Cobb Angle was better than the control group,and the difference was statistically significant(P<o.05).There were no internal fixation failures in either group.[Conclusion]:For the single segmental thoracolumbar compression fracture,the treatment of pedicle screw fixation can achieve the same clinical effect as the cross section Nail,and the long-term vertebral orthopedic effect is better than the trans-vertebral pedicle nail treatment.
