Objective:The breast cancer axillary lymph node dissection is a important part in breast cancer surgery,this paper discusses the independent factors of axillary lymph node metastasis and axillary lymph node metastasis.To provide a theoretical basis for the treatment of axillary lymph node,in the premise of ensuring efficacy and reduce unreasonable treatment of axillary lymph node,in order to reduce the unreasonable operation with pain.Methods:From March 2015 to December 2016 in our hospital of breast and thyroid surgery has been for axillary lymph node dissection(including partial axillary lymph node dissection and total axillary lymph node dissection)in 176 cases of breast cancer patients.Study the following factors: age,menarche age,fertility,age of first birth,menopause,menopausal age,course of disease,with or without previous history of benign breast no,a family history of malignant tumor,number of pregnancy,tumor location,tumor size,clinical axillary lymph nodes,obesity index,pathological type,pathological grade,estrogen receptor(ER),progesterone receptor(PR),Her-2 gene,Ki-67 gene,P53,E-Cadherin and so on.The above factors by single factor analysis(the chi square test)reached statistical significance index(P<0.05),the statistically significant indicators included in the regression model,using multivariate analysis(Logistic regression)were independent risk factors(receiver opera by ROC.Ting characteristic curve)curve was used to evaluate the efficacy of multivariate analysis(Logistic regression analysis).Results:Transfer the results of single factor analysis on axillary lymph node metastasis of axillary lymph nodes(neoadjuvant chemotherapy group)tumor size,course of disease,Her-2 gene,imaging of axillary lymph nodes and axillary lymph node metastasis(P<0.05),axillary lymph node metastasis(without neoadjuvant chemotherapy group)tumor size,duration,imaging axillary lymph nodes and axillary lymph node metastasis(P<0.05);axillary lymph node metastasis multivariate analysis(Logistic regression analysis)results: neoadjuvant chemotherapy group and no tumor size in neoadjuvant chemotherapy group,imaging of axillary lymph nodes and axillary lymph node metastasis was related to metastasis(P<0.05).Single factor analysis the results of group ? axillary lymph node: axillary lymph node metastasis(neoadjuvant chemotherapy group)of I + II group of lymph node metastasis,tumor size,lymph node metastasis in group I + II and ? group number of axillary lymph node metastasis(P<0.05),axillary lymph node metastasis(without neoadjuvant chemotherapy group)of I + II group of lymph node metastasis,tumor size,axillary lymph node imaging,I + II group number of lymph node metastasis,menopausal status and axillary lymph node metastasis of axillary ?(P<0.05);multivariate analysis of lymph node metastasis(Logistic regression)results: axillary lymph node metastasis(neoadjuvant chemotherapy group)of I + II group of lymph node metastasis and the number of axillary lymph node metastasis was significantly correlated(P<0.05),axillary lymph node metastasis(without neoadjuvant chemotherapy group)of the number of I + II group lymph node metastasis,tumor size and axillary lymph node metastasis was significantly correlated(P<0.05).The ROC curve showed that the multivariate analysis(Logistic regression analysis)was of clinical significance.Conclusion:1.The size of the tumor,the image of axillary lymph nodes and axillary lymph node metastasis was positively correlation.Tumor size,imaging axillary lymph node status is an independent factor of axillary lymph node metastasis.2.The number of I + II group lymph node metastasis,tumor size and axillary lymph node metastasis was positively correlation.The number of I + II group metastatic lymph nodes(3 or more)is independent risk factors of the axillary lymph node(neoadjuvant chemotherapy group).The number of I + II group metastatic lymph nodes(4 or more),tumor size is independent risk factors of axillary lymph node(without neoadjuvant chemotherapy group).
