| Objective1.To summarize the clinical data of patients with herpes zoster and explore the pathogenesis of herpes zoster.2.Tocompare the efficacy of three antiviral drugs:foscarnet sodium,ganciclovir,valacyclovir.3.To study the influencing factors of postherpetic neuralgia and provide reference for clinical diagnosis and treatments.4.To analysis 2 cases of initial diagnosis of herpes zoster and sum up clinical experience.MethodsThis study analyzed 112 patients with herpes zoster who were hospitalized in the ZhongDa Hospital Southeast University from September 2015 to January 2017 retrospectively.1.Recorded conditions of sex,age,characteristics of skin lesion,pain degree and so on,blood tests were performed in patients with biochemical,electrolyte,and serum troponin I.2.All patients were randomly divided into three groups,compared with the efficacy of foscarnet sodium,ganciclovir,valacyclovir.3.To explore the influencing factors of postherpetic neuralgia by multivariate analysis.4.Analyze and discuss the experience in 2 cases of initial diagnosis of herpes zosterResults1.In the 112 HZ patients,men were slightly more than women,74%of patients were older than 60 years;lesions mainly in the trigeminal nerve distribution area,intercostal nerve distribution area,lumbosacral nerve distribution area,the ratio between left and right was 1:1;the severity of skin lesions was;the average degree of pain in the acute phase of the VAS score of 4.96 points;29.5%of patients with complications,and most of the age of≥60 years old,PHN is the most complications;A total of 6 cases of special type HZ;average antiviral treatment days for 8.25 days;28.6%of patients with elevated triglycerides;Most patients had a calcium ion concentration between 2.20 and 2.29 mmol/L;3.6%of patients with elevated cTnI.2.There was no significant difference in the efficacy of these three antiviral drugs(F =0.239,P =0.788).3.The influencing factors of postherpetic neuralgia:Single factor statistical analysis showed that the difference was not statistically significant in the location,body side and cTnI,Age,lesion severity,pain score and electrolyte calcium concentration were statistically significant.Multivariate logistic regression analysis showed that Age,sex,severity of skin lesions,antiviral days and cTnI were not statistically significant;and the difference of pain score(P = 0.015)and electrolyte calcium ion concentration(P = 0.003)were statistically significant.The pain score is the risk factor of PHN,and that electrolyte calcium ion concentration is the protective factor of PHN.4.Diagnosis and treatment of herpes zoster,in addition to careful observation of skin damage,specialist examination,but also carefully asked about the history,observe the general situation,physical examination,some routine examination such as blood urine routine,biochemistry,electrolytes,troponin,myocardium Zymogram,electrocardiogram and other checks are also necessary.Conclusion1.foscarnet sodium,ganciclovir and valacyclovir can be effective in the treatment of herpes zoster,but we need to accord to the specific circumstances of the patient to choose different antiviral drugs,in order to reduce the risk of adverse reactions.2.PHN is a common complication of herpes zoster,herpes zoster acute severe pain,electrolyte calcium ion concentration in patients have more probable to occur postherpetic neuralgia.3.It is important for diagnosis and differential diagnosis of herpes zoster to do physical examination and three routine examination. |
