Objective:Uterine leiomyoma is the most common benign tumor in women.it occurs in women at reproductive age.They are the primary indication for hysterectomy,and are the cause of significant morbidity from profuse menstrual bleeding,pelvic discomfort and so on.Estrogen plays a critical role in the pathogenesis of human uterine leiomyoma.Therefore,researched differential expression of Estrogen metabolism enzymes(CYP1A1、COMT)and Estrogen receptor(ER)in uterine leiomyoma and its matched myometrium and the relationship between in them and investigated the role of CYP1A1、COMT、ER in the pathogenesis of the uterine leiomyoma.To explored the relationships between the CYP1A1、COMT、ER expression in uterine leiomyoma and clinic features.Methods:We collected the center tissue of the largest uterine leiomyoma and its matched myometrium from 18 patients with uterine leiomyoma who underwent Laparoscopic or laparoscopic hysterectomy in Shanghai TenthPeople’s Hospital from November 2016 to April 2017,except the patients who merged adenomyosis of uterus.All patients were not used the hormone therapy prior to surgery within 3 months,and Postoperative tissue was diagnosed with uterine leiomyoma.The age of the patients was within 41 and 57 years old,with the human uterine leiomyoma(HL)group as the experimental group,corresponding to the human myometrium(HM)as the control group.The mRNA expression of CYP1A1、COMT、ER in uterine leiomyoma and its matched myometrium was measured by reverse transcription-polymerase chain reaction(RT-PCR)in 18 cases of those specimens.The experimental data were obtained by Image Tool software.Statistical analysis was performed using SPSS for windows 20.0 software.Results: 1、Real-time PCR detected results show that:1.1 In the group of the human uterine leiomyoma(HL),the human myometrium(HM),CYP1A1 mRNA expression of relative quantity were 1.64±0.49、1,P<0.01.P is similar between the two groups was statistically significant.1.2 In the group of the human uterine leiomyoma(HL),the human myometrium(HM),COMT mRNA expression of relative quantity were 1.56±0.48、1,P<0.05.P is similar between the two groups was statistically significant.1.3 In the group of the human uterine leiomyoma(HL),the human myometrium(HM),ER mRNA expression of relative quantity were 4.73±1.39 、 1,P<0.001.P is similar between the two groups was statistically significant.2、the relationships between the CYP1A1、COMT、ER relative expression in uterine leiomyoma and clinic pathological features.2.1 A positive correlation of Age and CYP1A1 mRNA relative expression in uterine leiomyoma was shown by Spearman rank correlation coefficient(r=0.521,p<0.01),and A positive correlation of the largest leiomyoma size and CYP1A1 mRNA relative expression in uterine leiomyoma was shown by Spearman rank correlation coefficient(r=0.383,p=0.021)and CYP1A1 mRNA relative expression in uterine leiomyoma was significantly higher in postmenopausal samples than premenopausal samples by Mann-Whitney U test(8.624±11.184vs1.189±1.368,P<0.001).Spearman rank correlation also revealed no correlation between body mass index(BMI)、the largest leiomyoma size and CYP1A1 mRNA repression(P>0.05).CYP1A1 mRNA expression was not altered by anemia,hyper menorrhea,or the number of the leiomyoma type(P>0.05).2.2 A positive correlation of the largest leiomyoma size,Age and COMT mRNA relative expression in uterine leiomyoma was shown by Spearman rank correlation coefficient(r=0.357,p<0.05)、(r=0.360,p<0.05).Spearman rank correlation also revealed no correlation between body mass index(BMI)and ER m RNA repression(P>0.05).COMT mRNA expression was not altered by anemia,hyper menorrhea,menopause or the number of the leiomyoma type(P>0.05).2.3 A positive correlation of the largest leiomyoma size and ER mRNA relative expression in uterine leiomyoma was shown by Spearman rank correlation coefficient(r=0.750,p<0.001),and ER mRNA relative expression in uterine leiomyoma was significantly higher in postmenopausal samples than premenopausal samples by Mann-Whitney U test(7.092±4.898 vs 2.392±2.565,P<0.01).Spearman rank correlation also revealed no correlation between body mass index(BMI),Age and ER mRNA repression(P>0.05).ER m RNA expression was not altered by anemia,hyper menorrhea,or the number of the leiomyoma type(P>0.05).3、the relationship between the three3.1 Spearman rank correlation also revealed no correlation between mRNA relative repression and ER mRNA relative repression(P=0.05).3.2 A positive correlation of CYP1A1 mRNA and COMT mRNA relative expression was shown by Spearman rank correlation coefficient(r=0.471,P<0.01).3.3 Spearman rank correlation also revealed no correlation between CYP1A1 m RNA relative repression and ER mRNA relative repression(P>0.05).Conclusion:1、The expression of CYP1A1 mRNA、COMT mRNA、ER mRNA in uterine leiomyoma was significantly higher than in human myometrium.The result suggests that CYP1A1 、 COMT 、 ER may be associated with the onset of uterine leiomyoma.2 、 A positive correlation of the largest leiomyoma size and CYP1A1mRNA、COMT mRNA、ER mRNA relative expression in uterine leiomyoma was shown by Spearman rank correlation coefficient。The result suggests that CYP1A1、COMT、ER accelerates tumor growth.3、A positive correlation of Age and CYP1A1mRNA、COMT mRNA relative expression in uterine leiomyoma was shown by Spearman rank correlation coefficient.4、CYP1A1mRNA、ER mRNA relative expression in uterine leiomyoma was significantly higher in postmenopausal samples than premenopausal samples by Mann-Whitney U test.The result suggests that ER、CYP1A1 plays an important role in the continuous growth of postmenopausal uterine fibroids.5、A positive correlation of CYP1A1 mRNA and COMT mRNA relative expression was shown by Spearman rank correlation coefficient.The result suggests that CYP1A1 and COMT play a synergistic role in the development and progression of uterine fibroids.
