| ObjectiveTriple-negative breast cancer is a subtype of breast cancer with unique pathological features.It has strong invasiveness,and is easy to relapse and metastasize in the early stage.The prognosis is poor.This article summarizes the clinical features,pathological features,recurrence,metastasis,and prognosis of 90 patients with triple-negative breast cancer to investigate the pathological features and prognostic factors of triple-negative breast cancer.MethodsA retrospective analysis was performed on 815 patients of breast cancer treated in the Department of General Surgery of the First Affiliated Hospital of Nanchang University from January 2009 to December 2012.Among them,128 cases were triple-negative breast cancer.90 patients were selected according to the inclusion criteria and exclusion criteria,We collected the clinical data of these patients.Follow-up data were obtained through clinical follow-up and telephone follow-up.The time of recurrence,recurrence site,and recurrence-to-death time were recorded.All of them were followed up until the death of the patient or until the follow-up deadline.All data were analyzed using SPSS 20 for statistical analysis.Chi-square test was used for comparison of count data,and binary logistic regression analysis was used for multivariate analysis of treatment methods.Survival analysis was performed using the Kaplan-Meier method and Log-Rank test to compare survival rates,with p<0.05 being considered statistically significant.ResultsTriple-negative breast cancer accounted for 15.7% of all breast cancers,and 90 cases of triple-negative breast cancer patients ranged from 22 years old to 78 years old with an average age of 46.8 years old,of which 12 cases were 35 years old and 67 cases were 35 to 60 years old.11 cases were over 60 years old.62.2%(56/90)of patients were in pre-menopausal state.In 98.9% of the patients,the pathological type was invasive ductal carcinoma(89/90),of which pathological stage Ⅰ was 5 cases,stage Ⅱ 65 cases,stage Ⅲ 19 cases,stage Ⅳ 1 case.20 patients have recurrence and metastasis,and 17 of them died.all All patients who died were breast cancer-related deaths.The peak of recurrence was within 3 years after operation,accounting for 35% of the overall recurrence,and the median recurrence time was 23.83 months.The common first recurrence sites were bone(50% of all recurrences),lung(20% of all recurrences),and liver.(15% of all relapses),brain(15% of all relapses).The survival analysis showed that the 5-year overall recurrence-free survival rate was 77.78% and the 5-year overall survival rate was 88.01%.Multivariate analysis of COX regression revealed that pathological stage was the only independent prognostic factor associated with triple-negative breast cancer.ConclusionTriple-negative breast cancer has strong invasiveness,high metastasis,late diagnosis,early recurrence,rapid progression from recurrence to death,and high mortality.The prognosis of patients with stage Ⅰ and Ⅱ is better,and the prognosis is poor in stage Ⅲ and Ⅳ.Currently,Clinical treatment of triple-breast cancer are limited,on the other hand,Endocrine and anti-HER-2 therapy are ineffective,These lead to huge challenges in the diagnosis and treatment of triple-negative breast cancer.Early detection,early diagnosis and active search for targeted targets are still the focus of future clinical work and research. |
