Cancer-related fatigue is a symptom often occurring and difficult to self remission in the treatment of many malignant tumor patients.At present,there are still deficiencies in the understanding,the degree of attention and the pertinent intervention methods of medical care workers and patients.It is mainly related to the lack of CRF cognition and the lack of knowledge and skills for the effective management of CRF.This subject investigates and studies the deficiencies of the two aspects of the medical and nursing staff in cognition and practice.This topic is divided into two parts:a survey of nurses’ knowledge and attitude towards cancer-related fatigue and intervention of exercise prescription on cancer related fatigue in patients with gastrointestinal cancer undergoing chemotherapy.PART1:A survey of nurses’ knowledge and attitude towards cancer-related fatigueObjective:Cancer-related fatigue is the focus of clinical symptom management in cancer patients.This study investigated the cognitive status of CRF among nurses.Methods:a cross-sectional survey was used to investigate the general situation and the knowledge and attitude towards cancer related fatigue of nurses of three hospitals in Nanjing.Results:142 nurses received the questionnaire,the accuracy rate was 76.25%,and the nurses’ cognition of CRF was generally insufficient.The cognitive basis of knowledge of CRF oncology nurses ratio(84.30%)internal medicine,surgical nurses in other patients treated tumors(75.98%,79.57%),the difference was statistically significant(P<0.05).Nurses in various departments lack general knowledge about CRF management,and the difference is not statistically significant.Nurses have a strong attitude towards CRF treatment,and 94.36%of nurses agree that medical institutions should strengthen the management of CRF.Conclusion:at present,clinical nurses have positive attitudes towards CRF,but lack of related cognition and lack of training should be strengthened in future work.PART2:Intervention of exercise prescription on cancer related fatigue in patients with gastrointestinal cancer undergoing chemotherapyObjective:to apply individualized exercise prescription to chemotherapy patients with gastrointestinal cancer after cancer-related fatigue and observe the effect.Methods:78 gastrointestina cancer patients with CRF were selected and divided into experimental group and control group randomly.The control group was free to exercise.The experimental group was operated and supervised according to the individual’s exercise prescription.At first,fourth and sixth courses of chemotherapy,the brief fatigue scale(BIF),quality of life scale(QLQ-C30),Scored Patient-Generated Subjective Global Assessment(PG-SGA)、Appendicular Skeletal Muscle Mass(ASM)and IL-6 were measured.Results:The fatigue score in the experimental group was significantly lower than that in the control group in the fourth and sixth courses of chemotherapy(P<0.05).The fatigue scores of the two groups were statistically significant.The fatigue score of the control grorp increased significantly(P<0.05)while the intervention group fatigue score decreased significantly(P<0.05).Quality of life score:there was no significant difference between the cognitive function of the two groups before and after intervention(P>0.05).The emotional function,role function and social function of control group were significantly decreased(P<0.05 or P<0.01).The comparison between the groups showed that the role function,social function,body function and emotional function of the intervention group were significantly better than those of the control group(P<0.05).Nutrition score:the two groups of patients after the last chemotherapy compared with the nutritional score before intervention decreased,the difference was statistically significant(P<0.05).ASM:Before and after intervention,appendicular skeletal muscle increased significantly(P<0.05).IL-6:A significant increase of serum IL-6 was found(P<0.05)Conclusion:individualized exercise prescription can effectively relieve cancer-related fatigue and enhance muscle quality and quality of life in patients with gastrointestinal cancer undergoing chemotherapy.
