| Objective: To compare the effect of mini-external fixator and intramedullary nail fixation on high risk elderly patients with intertrochanteric fracture.Methods: From April2015 to July 2016,41 high risk elderly patients with intertrochanteric fracture of femur(15 males and 26 females)in the Orthopaedics Center of the first affiliated Hospital of Xinjiang Medical University were analyzed retrospectively.Two groups were divided into two groups,21 cases were treated with mini-external fixator,and 20 cases were treated with intramedullary nail fixation.The time of operation was evaluated in both groups.The difference of bleeding during operation,hospitalization,fracture healing and hip function score was analyzed by Hip Harris score.Results: 41 cases of high risk elderly patients were followed up for 6-18 months.The time of fracture healing in the external fixation group was shorter than that in the internal fixation group.The difference was statistically significant(P < 0.05).The number of bleeding in the external fixation group was significantly less than that in the internal fixation group,and the difference was statistically significant(P < 0.05).The postoperative complications in the external fixation group were significantly less than those in the internal fixation group,and the difference was statistically significant(P < 0.05).After follow-up,there was no significant difference in Harris score between external fixation group and internal fixation group(P > 0.05).Conclusion:Mini external fixator is superior to intramedullary fixation in the treatment of high risk elderly patients with intertrochanteric fracture.Its advantages are low risk of anesthesia,simple and rapid operation,short duration of operation,and less surgical trauma.There was no significant difference between the two fixation methods in Harrisscore of hip joint,such as less bleeding amount,no need for blood transfusion,short time for fracture healing,less complications after operation and short time for hospitalization. |
