| Objective:To investigate the serum 25-hydroxy vitamin D,betatrophin and chemerin in type 2 diabetes mellitus retinopathy(T2DR),and analyze corrlation among them.The purpose is to provide some new ideas for the early prevention and treatment of type 2 diabetic retinopathy.Methods:Selecting 120 cases of Type 2 Diabetes Mellitus(T2DM)treated by Department of Endocrinology of Affiliated Hospital of Zunyi Medical College from April 2016 to August 2017.T2 DM patients were divided into diabetic retinopathy(DR)Group(n = 60)and T2 DM non-DR group(n = 60).According to the American "Early Treatment Diabetic Retinopathy Study"(ETDRS)classification,the DR group was divided into 30 cases of non-proliferative diabetic retinopathy(NPDR),30 cases of proliferative diabetic retinopathy(PDR).In the same period,60 healthy subjects of matched ages were selected as normal control(NC)group.All subjects were tested for serum 25(OH)D,betatrophin,chemerin,fasting plasma glucose(FPG),glycosylated hemoglobin(Hb Alc),fasting plasma insulin(FINS),total cholesterol(TC),triglyeride(TG),high density lipoprotein-cholesterol(HDL-C)and low density lipoprotein-cholesterol(LDL-C).The differences of serum 25(OH)D,betatrophin,chemerin and related clinical indexes were compared between groups.Results:?Compared with NC group,25(OH)D,HDL-C levels were decreased,and betatrophin,chemerin,FPG,HbA1c,HOMA-IR,WHR,TC,TG,LDL-C levels were increased in T2 DM group(P<0.05).(2)Compared with NC group,25(OH)D,HDL-C levels were decreased,and betatrophin,chemerin,FPG,HbA1c,HOMA-IR,WHR,TC,TG,LDL-C levels were increased in NPDR group(P<0.05).?Compared with NC group,25(OH)D,HDL-C levels were decreased,and betatrophin,chemerin,FPG,HbA1c,HOMA-IR,WHR,TC,TG,LDL-C levels were increased in PDR group(P<0.05);(4)Compared with NPDR group,25(OH)D,HDL-C levels were increased,and betatrophin,chemerin,FPG,HbA1c,HOMA-IR,WHR,TC,TG,LDL-C were decreased in T2 DM group(P<0.05).(5)Compared with PDR group,25(OH)D,HDL-C level were increased,and betatrophin,chemerin,FPG,HbA1c,HOMA-IR,WHR,TC,TG,LDL-C levels were decreased in NPDR group(P<0.05).(6)Compared with PDR group,25(OH)D level was increased,the levels of Betatrophin,Chemerin,HbA1c,HOMA-IR,TC and TG in NPDR group were lower than those in PDR group(P<0.05),while the levels of WHR,LDL-C,HDL-C were not change significantly(P>0.05).Pearson correlation analysis showed that the level of 25(OH)D was negatively correlated with Betatrophin,Chemerin,Hb Alc,HOMA-IR,WHR,TC,TG and LDL-C,but positively correlated with HDL-C.Chemerin was positively correlated with Betatrophin,FPG,Hb Alc,HOMA-IR,WHR,TG and LDL-C,but negatively correlated with 25(OH)D and HDL-C.Betatrophin was positively correlated with Chemerin,Hb Alc,FPG,HOMA-IR,WHR,TG,LDL-C,and negatively correlated with 25(OH)D and HDL-C.Multiple linear regression analysis showed that 25(OH)D affecting factors were HOMA-IR,WHR,FPG and LDL-C;Betatrophin influencing factors were WHR and TG;Chemerin influencing factors were HOMA-IR,WHR and TG.Logistic regression analysis showed that 25(OH)D was protective factor in diabetic retinopathy,and HOMA-IR and HbA1c were the risk factors.Conclusion: The level of serum 25(OH)D in T2 DR is decreased,and the levels of Betatrophin and Chemerin are increased significantly.We speculated that they may be related to the development of DR. |
