Analysis The Causes Of Death Among People With Convulsive Epilepsy In Rural Ningxia From 2012 To 2016

Objective We analyzed the causes of death among convulsive epileptic patients in the epilepsy prevention and management program in rural Ningxia,and to provide a basis for policy of prevention and control for diseases.Methods Death data of convulsive epilepsy patients were collected in the epilepsy prevention and management program in rural Ningxia from January 1,2012 to December 31,2016.A uniform cause of death questionnaire was used to get death information.EpiData3.0and Excel2007 software was used to establish the death information databases,and to calculate the case fatality rate,annual average mortality,standard mortality ratio and proportion of death cause among epileptic patients.The distribution of death age,onset age,duration,cause of death and death location were analyzed.Results A total of 240 convulsive epileptic patients died in the epilepsy prevention and management program in rural Ningxia from 2012 to 2016,including 150 males(62.5%)and90 females(37.5%),and sex ratio was1.67: 1.There was no statistical difference in age distribution between men and women(p >0.05).The 5 years' case fatality rates were 1.41%,0.64%,1.21%,1.20% and 0.89% respectively.The annual average mortality was 1.54 per100000,the annual average mortality of males(1.92 per 100000)was significantly higher than the annual average mortality of females(1.16 per 100000).The risk of death in convulsive epileptic patients was 1.9 times that of the general population in rural areas of Ningxia.The death rate of all age groups has 2 peaks at 30-34 and 60-64 years.The higher onset age,the higher the proportion of deaths,there was no significant difference in the onset age and duration between men and women(p>0.05).The total causes of death included 88.8%of epilepsy-related causes of death and unrelated causes of epilepsy(or not related to the causes of death)11.3 %,Eepilepsy-related deaths accounted for the main body.Accidents(23.8%)ranked first,followed by status epilepticus(22.9%),sudden unexpected death in epilepsy(15%),stroke(12.9%)and cardiovascular disease(7.5%).Drowning was the main cause of death in accidents.Epilepsy-related causes of death included deaths directly due to epilepsy(64.2%),followed by deaths due to underlying neurologic disease(14.6%)and deaths indirectly due to epilepsy(12.5%),The average age of death in epilepsy-related causes of death was significantly different(p<0.01).The duration of epilepsy-related causes of death(<15y and ? 15y)was statistically significant(p<0.01).There were differences in the place of death(p <0.05).Conclusions 1.The risk of death in convulsive epileptic patients was 1.9 times that of the general population in rural areas of Ningxia.2.The composition ratio of death toll in each age group has double peaks,and the higher onset age,the higher the proportion of deaths.3.Epilepsy-related deaths accounted for the main body in the total causes of death,the top cause of death was accident,followed by status epilepticus,sudden death,stroke and cardiovascular disease.
