Objective To analyze the correlation between Hcy and left ventricular mass index in patients with H type hypertension and to explore the synergistic effect of hypertension and hyperhomocysteinemia on the formation of left ventricular hypertrophy in patients with H type hypertension.Methods A total of 122 patients with essential hypertension were selected from the Department of physical examination of Ningxia Medical University General Hospital.They were divided into H hypertension group and non H hypertension group according to whether they had hyperhomocysteinemia.The clinical data,biochemical indicators and drug use history of two groups were recorded.The left ventricular morphology and structure were measured by color Doppler ultrasound diagnostic system.The correlation between Hcy and left ventricular mass index in patients with type H hypertension was analyzed.Results(1)H type hypertension group and non hypertension H group in age,gender,height,weight,smoking history,homocysteine(Hcy),hematocrit(Hct),uric acid(UA),urea nitrogen(BUN),creatinine(Cr)were statistically significant differences in body mass index(P<0.05);diabetes,thrombocytocrit(Pct),fasting blood glucose(GLU),triglyceride(TG),low density lipoprotein(LDL-C),total cholesterol(TC)and high density lipoprotein(HDL-C)showed no significant difference(P > 0.05).The two groups had no statistically significant difference in the degree of hypertension,hypertension,antihypertensive therapy time,duration of untreated and grade product,and the number of antihypertensive drugs such as beta blockers;two groups in drug use,such as the difference of ACEI/ARB,calcium channelblockers,diuretics,anti platelet aggregation drugs,statins has significant differences;two patients were no spironolactone,folic acid and vitamin B and other drug use history.There was no significant difference between the two groups in the cardiac ultrasound EF,FS,LVMI,LVM,IVST,LVEDD,LVPWT and other indicators.(2)By eliminating the variation of the regression coefficient collinearity and VIF stepwise screening,X and clinical expertise do not need to be adjusted for covariates,the age,height,body mass index,gender,smoking,diabetes,Hct,HDL-C,TG,UA,GLU,Cr,antihypertensive treatment time,treatment time and the grade of product,not beta blockers,diuretics,antiplatelet drugs,statins,EF as a covariate in the regression equation,using Hcy as independent variables,using LVMI as the dependent variables for regression analysis.(3)in single factor regression analysis,gender and height were positively correlated with LVMI(r=12.386 P < 0.001,r=0.399 P < 0.05),heart rate and EF were negatively correlated with LVMI(r=—0.234 P < 0.05,r=—0.789 P < 0.01),and there was no correlation between the level of P and that of LVMI.In multiple regression analysis,adjusted for height,heart rate,EF and other covariates,gender and LVMI still positive correlation(r=19.970,P < 0.001);adjusted for sex and other covariates,height,heart rate,EF and LVMI between Hcy and LVMI disappeared,still no correlation.Conclusion(1)There was no correlation between high Hcy and left ventricular mass index in patients with essential hypertension.(2)sex is an independent risk factor for left ventricular hypertrophy in hypertension.
