| Objective Evaluate the short term clinical effects of posterior cruciate retaining total knee prostheses and posterior cruciate-stabilizing total knee prostheses in moderate and severe knee osteoarthritis and their effects on the quality of life after operation.Methods Follow-up the patients with knee osteoarthritis undergoing total knee arthroplasty operation from April 2017 to October 2017 in Department of Orthopedic Joint surgery,Lanzhou General Hospital of the Chinese People’s Liberation Army.According to the inclusion criteria and exclusion criteria,a total of 73 cases included 78 knees with complete data were selected.They were divided into posterior cruciate-retaining prostheses group(37 cases with 37 knees)and posterior cruciate stabilizing prostheses group(36 cases with 41 knees).The perioperative blood loss and knee flexion and extension activity of the two groups were counted.The score of visual analogue scale,hospital for special surgery,American knee society,Western Ontario and McMaster Universities osteoarthritis index and lower limb force line were used to evaluate the clinical effect of total knee arthroplasty.Patients’ health status were evaluated by 36-item short-form health survey.Results 73 cases with 78 knees were followed up.Perioperative blood loss was about(581.12±256.09)ml in posterior cruciate-retaining prostheses groups and(761.42±366.42)ml in posterior cruciate stabilizing prostheses groups.There was significant difference between two groups(P < 0.05).The range of motion results of one month after the operation were(93.95±7.82)degrees in posterior cruciate-retaining prostheses groups and(99.56±6.20)degrees in posterior cruciate stabilizing prostheses groups.There were significant statistical differences between the two groups(P <0.05).Western Ontario and McMaster Universities osteoarthritis index were(28.70 ± 5.99)score and(31.87 ± 7.00)score in two groups.There was significant difference between two groups(P < 0.05).However,there was no significant difference in knee flexion and extension activity at three months after operation(P >0.05).The score of visual analogue scale,hospital for special surgery,American knee society and lower limb force line were no the significant difference in the two groups(P >0.05).All the patients had no complications except 1 case with infection in other parts and 1 case with peroneal nerve injury occurred in the posterior cruciate stabilizing prostheses groups.Conclusion Total knee arthroplasty is an effective method for the treatment of moderate and severe knee osteoarthritis.Posterior cruciate-retaining total knee prostheses can reduced the amount of recessive blood loss in perioperative period and preserved the original sensation of the knee joint,which can obviously improve the patients’ ability to engage in daily life after operation.Early knee flexion and extension activity was obviously improved in posterior cruciate stabilizing total knee prostheses.However,in third month,the flexion and extension of knee is not significant difference.Both types of prostheses can relieve joint pain,improve joint function and recovery lower limb force line.Both of them have the same effect on the physiology and psychology of the patients after operation. |
