| Objective Through the study of the efficacy of different treatment methods for different types of proximal humerus fracture, to explore the optimal treatment of different types of fractures of the proximal humerus.Methods Analyze informat ion on82cases of proximal humeral fractures us ing the Neer class if icat ion, treatment methods, including manual reset neck wrist straps or sling suspension or splint, closed reduction by skin pinning incision reduction and internal fixation of humeral headprosthetic replacement of four kinds of follow-up to assess the treatment effect, thus the optimal solution to this type of fracture.Results All patients were followed up for6to12months, an average of seven months. Assessment shoulder function score according to Constant and Murley shoulder score in this group the results of conservative treatment group, the total rate of90.62%, closed the reset needle group of excellent and good rate of80.00%, cut the excellent and good rate of reduction and plate fixation group,77.78%of the humeral head prosthesis replacement group rate of75.00%. The efficacy of conservative treatment group was significantly stronger than the other groups.Conclusion For proximal humeral fractures should be based on the different types, using different approaches to treatment. Part of the overall fracture due to the priority of conservative treatment, two-part fractures in addition to some fractures used by skin Kirschner wire fixation, conservative treatment is still the main options being considered, the three-part fractures with open reduction and plate screw fixation become a major treatment options, four-part fractures in young patients advocate open reduction and internal fixation surgery in the fight for the greatest degree of recovery of shoulder function in patients with the premise, and does not affect the shoulder joint function and range of activities recommended in the rehabilitation phase on the basis of early activity. Shoulder arthroplasty in elderly patients or severe proximal humerus comminuted fracture can not be used fixation techniques, should be selected, while for patients restricted to their own physical condition, and careful consideration in the choice of treatment. |
