ObjectiveThrough the clinical observation of acupuncture combined with grain-sized moxibustion on the treatment of chronic eczema and evaluating its validity, feasibility, safety and long-term efficacy,it was aimed to find a new way to enrich the acupuncture treatment on eczema and serve the clinic better.Through observation and statistics on the distribution of eczema,it was aimed to provide a certain point selection strategy for acupuncture treatment of eczema.MethodsThirty patients who met the inclusion criteria were randomly divided into observation group and control group by random number table method.Among them,15 cases were observation group,15 cases were control group(falling off 1 case),and 29 cases were actually observed. The treatment points were acupuncture points based on the conventional acupoint selection plus the skin lesion sensitive points. The conventional acupoints in two groups were both treated with filiform needle acupuncture, mild reinforcing and reducing, and retaining the needle for 20 minutes. At the same time, the skin lesion sensitive points in the observation group were treated with grain-sized moxibustion, while that in the control group were treated with filiform needle acupuncture, mild reinforcing and reducing, and needle retention for 20 minutes. Both groups were treated twice a week, and 16 times totally for this treatment.Observe and record the VAS score and EASI score which were used to assess the pruritic degree of eczema, and the area and severity of eczema lesions before treatment and after 2th,4th,8th,12th and 16th treatment.Compare the recurrence and aggravation between the two groups one month after the end of treatment. All recorded data were analyzed with SPSS22.0 statistical software.Results1)There were no significant differences between the two groups in terms of the general conditions, the degree of itching(VAS score),the lesion area and severity(EASI score)before treatment(P>0.05),which means the two groups were comparable.2)Comparing the two groups of general therapeutic effect, the total effective rate was 93.3%in the observation group, and 78.6%in the control group. There was no significant difference in the total effective rate between the two groups by chi-square test(P>0.05),but the total score reduction rate was statistically significant by t-test(P<0.05),indicating that the observation group was better than the control group in the general therapeutic effect.3)Comparing the pruritic degree in the two groups, the difference in VAS scores after treatment between the two groups was statistically significant(P<0.05),indicating that the observation group was better than the control group in the treatment of itching.4)Comparing the lesion area and severity of the two groups, there was no significant difference in the EASI score between the observation group and the control group after the treatment(P>0.05).5)All scores of before and after treatment were compared within each group, and the differences were statistically significant(P<0.05),indicating that both treatments were effective.6)The distribution rules of chronic eczema:it’s meridian circulations were partial to the yang meridian, especially on meridians of foot-yangming,foot-shaoyang,foot-jueyin,foot taiyang,foot-taiyin,and hand-shaoyang;it’s spinal nerve cord segments were dominated on L5,S1,L4,C6 and C7.Conclusion1)Grain-sized moxibustion and acupuncture both had therapeutic effects on chronic eczema.Grain-grained moxibustion was superior to acupuncture in improving the total curative effect of chronic eczema and relieving itching with a faster effect. While grain-sized moxibustion did not have the advantage on the improvement of lesions compared with acupuncture.2)The long-term efficacy of grain-sized moxibustion in the treatment of chronic eczema was acceptable. And it was safe and reliable with few adverse reactions.
