Study On The Distribution And Treatment Of TCM Syndrome Elements In Ventricular Premature Beats

Objective:To explore the characteristics of traditional Chinese medicine(TCM)syndromes and the combination of syndrome of premature ventricular beats(PVB),and the rule of clinical medication.Methods:1.Literature research:Through searching the clinical literature related to PVB published in China from 2007 to 2017,the syndromes of the cases included in the literature were analyzed by SPSS 22.0,and to sum up the TCM syndrome factors and the rules of combination of the syndromes of PVB.2.Clinical study:The study included 147 patients with PVB who were admitted to The Department of cardiovascular medicine of Guang’an men Hospital from January 2014 to June 2017.Clinical symptoms and tongue pulse,TCM syndromes,and treatment methods were collected.The SPSS 22.0 and traditional Chinese medicine supporting platform system were used for statistical analysis and data mining of TCM syndrome elements and combination forms of PVB and prescriptions of traditional Chinese medicine formulas.Results:1.Through searching of domestic databases,111 articles relating to TCM syndromes of PVB were eventually included.A total of 9,709 cases involving 12 kinds of syndrome elements.The top three proportions were qi deficiency,yin deficiency,and blood stasis.A total of 32 syndrome combinations,including 6 single-factor syndromes,were ranked according to the proportion of blood stasis>yang deficiency>qi deficiency>internal fire,and 13 combinations of two-factor syndromes,qi deficiency + yin deficiency,qi deficiency +blood stasis,qi deficiency + blood deficiency,phlegm + blood stasis,yin deficiency +internal fire,8 kinds of three-factor combination syndrome,qi deficiency + yin deficiency +blood stasis,qi deficiency + phlegm + internal fire,and 4 kinds of four factors combination syndrome,the most of which is qi deficiency + blood stasis + yin deficiency + phlegm,and one combination of five factors:qi deficiency + blood deficiency + blood stasis + internal fire+ internal dampness.2.Clinical study included 147 patients with PVB who were hospitalized at the Guang’an men Hospital.There were 37 types of TCM syndrome differentiation types.The top five were:qi and yin deficiency syndrome,phlegm and blood stasis syndrome,and qi deficiency and blood stasis,syndrome of internal resistance of phlegm dampness,and phlegm heat syndrome;divided into syndrome elements include a total of 11 kinds,deficiency syndrome followed by:qi deficiency(76 cases,51.7%)>yin deficiency(64 cases,43.5%)>blood deficiency(6 cases.4.1%)>yang deficiency(5 cases,3.4%),and the three cases in the front of the excess syndrome are blood stasis(78 cases,53.1%),phlegm(74 cases,50.3%),internal fire(27 cases;18.4%),the disease location was mainly heart-related and correlated with spleen,liver,kidney,stomach and other organs;the syndrome combinations were divided into single-factor syndromes(13 cases,8.8%),two-factor combination certificate(69 cases,46.9%),three-factor combination certificate(34 cases,23.1%),four-factor combination certificate(31 cases.21.1%),mainly in multiple combinations,mainly for qi and yin Deficiency + other syndromes,phlegm and blood stasis + other syndromes.3.The clinical use of treatment of PVB in a total of 150 kinds of traditional Chinese medicine,through the TCM inheritance platform for statistical analysis,there are 34 commonly used drugs,mainly Poria(Puling),Pinellia(Banxia).Angelica(Danggui),Salvia Miltiorrhiza(Danshen),Radix Ophiopogon(Maidong),Radix Rehmanniae(Shengdi),Codonopsis(Dangshen)and so on.4.Through data mining,14 core drug combinations were obtained according to the association rules,including the Pinellia(Banxia)and Poria(Fuling),Keel(Longgu)and Oysters(Muli),Pinellia(Banxia)and Trichosanthes(Guolou),Pinellia(Banxia)and Dried Tangerine Peel(Chenpi),Radix Ophiopogon(Maidong)and Radix Rehmanniae(Shengdi).According to the clustering algorithm,the combination of the drug combination has the method of Pinellia(Banxia)and Fructus Aurantii(Zhiqiao),Rehmannia(Shengdi)and Radix Paeoniae Rubra(Chisaho),Dried Tangerine Peel(Chenpi)and Trichosanthes(Guolou).32 combinations were obtained by complex system entropy clustering.Angelica(Danggui)+Radix Paeoniae Rubra(Chishao)+ Peach Kernel(Taoren)+ Safflower(Honghua):Ligusticum Wallichii(Chuanxiong)+ Radix Paeoniae Rubra(Chishao)+ Peach Kernel(Taoren)+ Safflower(Honghua),Codonopsis(Dangshen)+ Cassia Twig(Guizhi)＋Gardenia(Zhizi),Keel(Longgu)and Oysters(Muli),Suanzaoren + Polygalaceae(Yuanzhi)+Baiziren,Fructus Corni(Shanzhuyu)+ Rhizoma Dioscoreae(Shanyao)+ Alisma(Zexie),Dried Tangerine Peel(Chenpi)+ Pinellia(Banxia)+ Radix Ophiopogon(Maidong)+ Radix Rehmanniae(Shengdi),Gastrodia(Tianma)+ Eucommia(Duzhong)+ Loranthaceae(Sangjisheng)+ Radix Polygoni Multiflori(Shouwuteng);According to the entropy hierarchical clustering to obtain 16 new combinations of PVB,such as Tangerine Peel(Chenpi)+ Pinellia(Banxia)+ Poria(Fuling)+ Radix Ophiopogon(Maidong)+ Radix Rehmanniae(Shengdi),Baikal Skullcap(Huangqin)+ Angelica(Danggui)+ Radix Paeoniae Rubra(Chishao)+ Peach Kernel(Taoren)+ Safflower(Honghua)+ Keel(Longgu),Ligusticum Wallichii(Chuanxiong)+ Radix Paeoniae Rubra(Chishao)+ Fructus Aurantii(Zhiqiao)+ Rhizoma Cyperi(Xiangfu)+ Peach Kernel(Taoren)+ Safflower(Honghua),Prepared Rehmannia Root(Shudi)+ Fructus Corni(Shanzhuyu)+ Rhizoma Dioscoreae(Shanyao)+ Alisma(Zexie),Asparagus Cochinchinensis(Tiandong)+ Lophatherum Gracile(Danzhuye)+ Polygalaceae(Yuanzhi)+ Suanzaoren + Baiziren,Keel(Longgu)and Oysters(Muli)+ Bombyx Batryticatus(Jiangcan)+ Cassia Twig(Guizhi).Conclusions:1.Through literature retrieval and clinical research,the main factors of TCM syndrome of PVB are Qi deficiency,yin deficiency,blood stasis and phlegm.In addition,blood deficiency is one of the main syndromes in the results of literature retrieval.Among the syndromes of clinical case mining and sorting,the proportion of internal fire is more.The pattern of syndromes and syndromes is composed of replete and vacuous factors,and is mainly composed of multiple syndromes.The main manifestations are the symptoms of Qi and Yin deficiency and other syndrome factors,phlegm and stasis syndrome + other syndrome factors,this result provides new ideas for clinical treatment of PVB in TCM syndrome differentiation.2.The Department of Cardiology of Guang’an men Hospital uses traditional Chinese medicine to treat PVB mainly to nourish yin,dispel phlegm,and promoting blood circulation for removing blood stasis;Through data mining,the core combination of common drugs and new prescriptions were obtained,which could guide the treatment of clinical medication better.
