Backgroud:Essential Hypertension is a global disease which is the most important risk factor for cardiovascular and cerebrovascular diseases.In China,with the improvement of economic and living standards,the morbidity has risen,but its awareness rate,treatment rate and control rate are not optimistic.Prolonged high blood pressure can cause damage to the heart’s structure and function,and even to heart failure.The high incidence of hypertension and the damaging to target organs such as hypertensive heart disease have become a major public health problem,causing a great economic burden.In recent years,with the development of traditional Chinese medicine,the advantages of prevention and control of chronic diseases have gradually emerged,and great progress has also been made in the treatment of hypertension.Treatment based on syndrome differentiation is the core of traditional Chinese medicine theory as well as the basis for ensuring efficacy.Based on this,to provide an objective and macro reference basis for TCM syndrome differentiation of hypertension has become a research hotspot.Ballistocardiography(BCG)is a new type of cardiac function monitoring method in the field of medical engineering,and research has proved it is possible to become a new cardiac function assessment method.Therefore,the author introduced this equipment into clinical research to explore the correlation between the main indicators of BCG and the different syndromes of TCM as well as the influence of cardiac function,aiming at exploring an objective basis for the differentiation of symptoms and signs for classification of syndrome of hypertension and improve the clinical effect of hypertension.Objective:1.To explore the differences between the main evaluation indicators of BCG in the changes of heart function between the hypertension group and the non-hypertension group,and to explore the feasibility of ECG in the evaluation of cardiac systolic function;2.To explore the differences and correlations between the main indicators of BCG in each syndrome type of hypertension.Methods:1.A total of 108 hypertension patients from May 2017 to December 2017 admitted to our study and included in the hypertension group;30 patients with age-matched exclusion of hypertension,coronary heart disease,and severe arrhythmia were selected as non-hypertensive group;2.The diagnostic criteria for hypertension refer to the "Guideline for Hypertension Treatment in China in 2010".The diagnostic criteria for hypertension in TCM according to the 2002 edition of the Guiding Principles for the Clinical Study of New Chinese Medicine,which divided into the following categories:ganhuokangsheng,yinxuyangkang,yinyangliangxu,tanshiyongsheng.Patients admitted to hospital after a definite diagnosis by at least one paraplegia and attendance were included in the hypertension and non-hypertension group according to the inclusion exclusion criteria;patients in the hypertension group were divided to the type IV criteria;3.Collecting general information and LVEF,NT-proBNP and other indicators for each group of patients.BCG were collected and main indicators measured;4.SPASS20.0 was used for statistical analysis of all data and conclusions were reached.Results:1.A total of 138 patients were included in the study,including 108 in the hypertension group and 30 in the non-hypertensive group.There was no significant difference in the general data between gender,age,and BMI between the two groups(P>0.05);2.The LVEF and NT-proBNP were different between the non-hypertension group and hypertension group(P<0.01),and the amplitudes of IJ/N and JK/V were significantly different(P<0.01);3.There was a significant difference in IJ/V among the TCM syndromes of hypertension(P<0.01).From the highest to the lowest,the IJ values were in the order of ganhuokangsheng,yinxuyangkang,tanshiyongsheng,yinyangliangxu.There were significant differences between any two groups(P<0.01);4.There was a significant difference in JK/V between the TCM syndromes of hypertension(P<0.01).The amplitude of JK from high to low was in the order of ganhuokangsheng,yinxuyangkang,tanshiyongsheng,yinyangliangxu.There was a significant difference in JK amplitude(P<0.01);5.There was no significant difference in tIJ,tJK,and tRJ(ms)among the TCM syndromes of hypertension(P>0.01);6.The number of LVEF exist difference between the TCM syndrome types of hypertension(P<0.01).LVEF showed a gradually decreasing trend.Yinyangliangxu is the lowest(P<0.01).There was no statistically significant difference between others except yinyangliangxu(P>0.01);7.There exist significant difference in NT-proBNP among the TCM syndromes of hypertension(P<0.01).NT-proBNP showed an upward trend in the type of four,and yinyangliangxu higher than others(P<0.01).There was no significant difference between the two other groups besides yinyangliangxu(P>0.01);8.The TCM syndrome type of hypertension is related to LVEF,NT-proBNP,IJ/V,and JK/V.With the changing of syndrome pattern,LVEF,IJ/V,and JK/V gradually decrease,and NT-proBNP gradually increases.Conclusion:1.There were significant differences in IJ/V and JK/V between the non-hypertension group and the hypertension group.IJ/V and JK/V were used in the evaluation of hypertension compared with the main indicators of other factors,which using to evaluate impairment of heart function has feasibility;2.IJ/V,JK/V,LVEF and NT-proBNP,all can reflect the chronic process of hypertensive heart damage;3.The changes of IJ/V and JK/V and TCM syndromes of hypertension are all related to each other,which can provide an objective basis for TCM syndrome types of hypertension.
