| Bronchial asthma is one of the most common chronic respiratory diseases,belonging to “asthma",“dyspnea",“cough" and“fluid retention" categories of TCM.At present,there are about 300 million patients in the world,and the number of asthma patients is rising year by year.Adrenocortical hormone is the first choice of drugs in the treatment of asthma,but because there are many adverse reactions in clinical use of hormone,it is more important to study the treatment of asthma in traditional Chinese medicine.Acupoint injection therapy is a characteristic therapy of traditional Chinese medicine which infuses the corresponding medicine into the fixed acupoints of the body.Chuankezhi Injection is mainly composed by Yin Yang Huo and Ba Ji Tian,which functions are warming the kidney and boosting the Yang energy,while relieving cough and asthma.So,it is a simple and easy way to improve the curative effect by combining Chuankezhi Injection with acupoint injection in clinic.It is significant to study the efficacy of different administration methods of Chuankezhi Injection in acute attack of asthma.Objective:In this study,a randomized controlled trial was conducted to compare the higher efficacy between the acupoint injection and the intramuscular injection of Chuankezhi Injection,in the treatment of acute exacerbation of bronchial asthma,for a better guidance of clinical medication.Methods:The inpatients with acute attack of bronchial asthma were randomly divided into two groups:the control group and the experimental group.The control group was given intramuscular injections of 4ml Chuankezhi Injection bid,on the gluteus maximus for 7 days on the basis of routine diagnosis and treatment of traditional Chinese and western medicine.The experimental group was treated with 2ml of Chuankezhi injection,by injecting into both sides of Shenshu acupoint alternately for 7 days on the basis of routine Chinese and western medicine diagnosis and treatment.The changes of TCM syndromes score,daytime and nocturnal score,pulmonary function tests and other indexes were compared between the two groups at admission and 7 days after admission.Result:1.Comparison of the total curative effect between the experimental group and the control group(P = 0.141).2.After treatment,the P value of scores of TCM syndromes of the two groups were less than 0.05 in comparison with those before treatment.The P value of inter-group comparison of scores of asthma,cough,sputum,chest tightness and wheezing before treatment were 0.111,0.112,0.906,0.750 and 0.307 respectively.After receiving treatment,the inter-group comparison P values of wheezing and chest tightness were 0.014 and 0.041 respectively,and the inter-group comparison P values of other symptoms and signs were more than 0.05.3.The P values of comparison between groups of nocturia and sore-cold sensation in the low back and knee before treatment of both groups were greater than 0.05,the comparison of P in both groups before and after the treatment of nocturia was more than 0.05.After treatment,the P value of nocturia conditions of inter-group comparison was 0.381.The control group’s P value inter-group comparison before and after treatment of sore-cold sensation in the low back and knee was 0.596,and the experimental group’s P value of that was 0.079.The P value comparison between both groups after treatment was 0.0324.The P values of comparison between both groups after treatment of nocturnal scores was 0.018,ACQ6 was 0.016,eosinophils cells count was 0.017,while in improving daytime scores and FEV1%,the P values of inter-group comparison were both 0.032.5.Comparing the relevance between the course of disease and medication,the average course of disease for the 21 patients who received medication regularly was 26.98 ± 18.55 years,the average course of disease for the 42 patients who did not was 16.86 ±16.856 years.The P value comparison between those who received medication regularly and did not,was 0.021.6.Bronchial asthma had a physical limitation score of 16.6 ±3.63,and emotional disorder score of 11.78 ±3.19,social limitation score of 12.11 ±3.16,and a health concern score of 12.40 ±3.14.Conclusion:The acupoint injection of Chuankezhi Injection is more effective than intramuscular injection to some extent,mainly in improving the symptoms of wheezing and chest tightness,nocturnal symptoms,ACQ6 score and so on.In terms of improving cough,sputum,wheezing,daytime score,pulmonary function tests,accupoint injection of a small dosage of Chuankezhi may be equivalent to that of conventional-dose. |
