| Objective:Myofascial pain syndrome is a condition of chronic muscle pain caused by myofascial trigger points.At present,due to the accelerated pace of the society and the widespread use of electronic devices,the incidence rate is rising.It continues to affect people’s lives and work,and causes a large amount of medical resources to be consumed.In this experiment,we observed the effects of needle-knife intervention on the expression of substance P,bradykinin,and calcitonin gene-related peptides in the triggering point of the medial muscle of the myofascial trigger point model rats.The influence of the local energy metabolism cycle at the fascia trigger point.Methods:Thirty-two adult healthy SD rats were randomly divided into four groups according to the random number table,blank group,model group,acupotome group,and injection group,with 8 rats in each group.An animal model of the triggering site of the medial muscular fascia was established using the ’Combat and Combine Eccentric Method’ and the model was performed once a week for a total of 8 weeks.At the end of the 8th modeling,each rat in the model group,the acupotome group and the injection group was subjected to a tenderness threshold test to check whether the model was successful or not.After the model was completed,the blank group of the model group were all raised normally and did not participate in the intervention.Rats in the injection group and the needle-knife group began to intervene one week after the model was successfully established.Intervene once a week for a total of 3 times.The intervention method was as follows:Acupotome group:prepare skin,disinfect the iodophor,and carefully search the trigger point with the pulp in the medial femoral muscle of the rat.Use a 0.25mm x 40mm acupotome.The knife edge line is parallel to the direction of the muscle fibers of the medial femoral muscle.Needle The knife body is perpendicular to the skin,piercing the tension band,cutting the trigger point 2 to 3 blades longitudinally,and applying pressure to stop the bleeding.Injecting group:Prepare the skin,disinfect the iodophor,and carefully search the trigger point with the pulp in the medial femoral muscle of the rat.Inject a 0.5ml 1%lidocaine into the trigger point with a 1ml syringe.The histopathological changes of myofascial tissue in the myofascial trigger points of rats were observed by HE staining.The expression of substance P,bradykinin,and calcitonin gene-related peptides in local muscles of myofascial trigger points were observed by immunohistochemistry.,and statistical analysis of differences between groupsResults:1.The results of the tenderness threshold test showed that before the intervention,the tenderness thresholds of the model group,the needle knife group and the injection group all decreased,and there was no significant difference between the groups(P>0.05).There was a significant difference between the two groups(P>0.05).<0.05)After the intervention,the tenderness threshold of needle-knife group and injection group was significantly higher than before intervention(P<0.05),and there was a significant difference compared with the model group after intervention(P<0.05).2.HE staining showed that the shape of muscle cells in the blank group was regular,the order of muscle fibers was orderly,and the thickness of muscle fibers was uniform;the thickness of the muscle fibers of the model group was different,and the muscle fibers of the model group were disordered,some muscle fibers were broken,and some of the muscle fibers that had been degenerate and contracted appeared;The muscle fibers of the broken ends are connected by connective tissue,and newly generated capillaries exist;the muscle fibers of the new needle-knife group are relatively mature,and some muscle fibers are still distorted,and newly formed capillaries exist.3.The results of immunohistochemistry showed that the expression levels of substance P,bradykinin,and calcitonin gene-related peptides in the model group were significantly higher than those in the blank group(P<0.05).Compared with the model group,the expression levels of substance P,bradykinin,and calcitonin gene-related peptide were significantly decreased in the acupotome group and the injection group(P<0.05).Compared to the injection group,the substance P and bradykinin were decreased in the acupotomy group.The expression of calcitonin gene-related peptide was significantly decreased(P<0.05).It shows that after the intervention of the acupotome,the release of substance P,bradykinin and calcitonin gene-related peptide in the muscle tissue at the trigger point is significantly reduced,and the effect is better than that of lidocaine injection.Conclusions:Acupotomy can cut the muscular tissue of local adhesions contraction at the trigger point of myofascial muscle,which is beneficial to the recovery of local muscle fiber shape and improve blood circulation.It can promote the metabolism of substance P,bradykinin and calcitonin gene-related peptide in the local tissue at the trigger point,which is beneficial for pain relief.Needle knife intervention is better than local injection of lidocaine. |
