Relationships Between HBV-M Expression Patterns,HBV-DNA And ALT Levels In Patients With Chronic HBV Infection Of Different Stages

Objective: To investigate the changes of HBV-M expression pattern,HBV-DNA load and ALT level in serum of patients with ASC,CHB,LC and HCC,and to provide clinical evidence for diagnosis,follow-up and early prediction of disease progression in patients with chronic HBV infection.Methods: To recall that 392 cases of chronic HBV infection were treated in our hospital between November 2014 and November 2016,which were grouped according to the different stages of the disease,and were eventually covered by the study markers: 82 cases of HBV carry group(ASC),88 cases of chronic B viral hepatitis group(CHB),86 cases of hepatitis B cirrhosis group(LC)and 78 cases of hepatitis B associated liver cancer group(HCC).To collect the clinical data of HBV-M,HBV-DNA and ALT level of serum and analyze the relationship.Results:(1)Gender and age among four groups were statistically significant difference(P<0.05).(2)HBVM expression patterns among the four groups was statistically significant difference(P<0.05).HCC group was significant statistically difference in comparing respectively to ASC,CHB groups(p<0.05).The pattern with HBeAg negative was the main expression pattern in HCC group,the rate of HBeAg negative expression was significantly higher than that of the previous two groups,there was no statistically significant difference between LC group;(3)HBV DNA replication level among the four puoups was statistically significant difference(P<0.05).HCC group respectively with ASC and CHB group's difference was statistically significant(P<0.05),HCC group of HBV DNA replication level was significantly lower than that of the ASC and CHB groups;LC group of HBV DNA replication level was significantly lower than that of the CHB group.There was no statistically significant difference in the rest of groups about HBV DNA replication level(P>0.05).(4)The difference of abnormal rate of ALT>2ULN among the four groups was statistically significant difference(P<0.05).After comparison,the difference of abnormal rate of ALT>2 ULN in CHB group was statistically significant(P<0.05).There was significant difference in ALT level among four groups(P<0.05).After comparison,the difference between CHB,ASC,LC and HCC group was statistically significant(P < 0.05).(5)The ALT levels of ASC,CHB and LC were correlated with HBV-DNA(P<0.05).In HCC group,ALT level and HBV-DNA load have no correlation(P>0.05).Under the condition of HBeAg positive expression,In ASC,LC,and HCC groups,ALT level and HBV-DNA load were on a numerical significant positive correlation(P<0.05).CHB group's ALT level and HBV-DNA load were no correlation.Under the negative expression of HBeAg,except the HCC group,the ALT of all the other groups was positively correlated with the load of HBV-DNA.Conclusion: 1.Hepatitis B cirrhosis and hepatitis B associated hepatocellular carcinoma were characterized by middle age male,HBeAg negative,low HBV-DNA load and no significant liver damage 2.HBV-M,HBV-DNA,and ALT in patients with chronic HBV infection was closely related to the development,the distinction between chronic HBV infection disease under the premise,combined with the HBV-M expression pattern changes,there was a good correlation between HBV-DNA and ALT,but can not reflect the evolution of the disease,only regular long time monitoring can be more objective and accurate evaluation of the condition changes and the severity of changes in patient management and to find the early state of the virus.
