Objective:To review clinical cases and analyze the characteristics and clinical application values of reduction mammaplasty with central gland pedicle technique in the treatment of hypermastia.Methods:A retrospective analysis was undertaken of 47 cases of patients(94 breasts)with hypermastia treated by the reduction mammaplasty with central gland pedicle technique at our department during 2004 and 2015.35 cases are married patients and 12 cases are unmarried and nonporous patients.Three localization methods were synthesized to determine the new location of the nipples.Double-circle method was used to conduct incision and tumescent technique was used to remove the epidermis between two circles to form a dermis cap.A separation was conducted along the surface of mammary to the base of the breast.The upper and lower or surrounding breast tissues were subjected to wedge resections to form a central gland pedicle,which was fixed to the chest wall and subjected shaping.The non-absorbable suture line was used for purse-string suture within the dermis of the outer circle incision.Preoperative design and intraoperative operation main points of this technique were elaborated.Objective and subjective evaluation of the 47 cases of patients were conducted.The characteristics,advantages and disadvantages of this technique were analyzed.Results:Among the 47 patients,there were 3 cases with 3 sides occurring incision adiponecrosis and liquefaction postoperatively and having a good incision healing after changing dressings and conducting drainage.The left patients were all in stage I healing.Patients’ subjective evaluation of pains brought by shoulders,neck,back and bra groove achieved a 100%relief,mobility inconvenience and produced mental burden received a 100%improvement,the inframammary fold eczema improvement rate was 88.9%and supine sleep quality improvement rate was 92.6%.In the objective evaluation,the bilateral breast sizes,nipple positions and areola size were all substantially symmetrical.Conclusion:The reduction mammaplasty with central gland pedicle technique is currently a fairly complete surgical method for the treatment of hypermastia.
