Objective:To observe application of dexmedetomidine in pediatric undergoing laparoscopic surgery for indirect inguinal hernia with general anesthesia research.Method:A total of 150 patients completed study,the pediatric undergoing of laparoscopic surgery indirect inguinal hernia with general anesthesia were randomly allocated to three group with 50 cases each:group D(dexmedetomidine lug/kg),group M(midazolam 0.1mg/kg)and group C(control group).Using the micro pump intravenous continuous infusion of sedative drugs,pumping injection time for 15 minutes.Pumping injection time for 10 minutes,begin to routine anesthesia and tracheal intubation.Recorded the SBP and HR pre-anesthetic(TO),after induction(T1),when tracheal intubation(T2),after tracheal intubation 5 min(T3),when tracheal extubation(T4)and after tracheal extubation 5 min(T5).Recorded the Sp02 and RR pre-anesthetic(TO),when tracheal extubation(T4)and after tracheal extubation 5 min(T5).The operation time and extubation time yet were recorded.The incidence rate of hypertension,hypotension,tachycardia,bradyarrhythmia,respiratory depression,hypoxemia,nausea and emesis were recorded after pulling off the cannulate.Evaluation of three groups of children with preoperative anxiety score,preoperative behavioral pain score,restless postoperative recovery and the in cidence of emergence agitation.Result:1.There was no significant difference in general datas for three group of patients(P>0.05).2.The extubation time of group D was longer than the other two groups(P<0.05).3.(1)SBP changes:The group D at T2 and T3 were lower than group C(P<0.05).The group D at T4 and T5 were lower than group D and group C(P<0.05).Intra-group comparison:Compared with TO,the group D,group M and group C at T1 were lower(P<0.05).The group D at T4 was higher(P<0.05).The group C at T4 and T5 were higher(P<0.05).(2)HR changes:The group D at T2,T3,T4 and T5 were lower than group M and group C(P<0.05).Intra-group comparison:Compared with TO,the group D and M at T1 and T3 were lower(P<0.05).The group C at T1 was lower(P<0.05)and T2,T4 and T5 were higher(P<0.05).4.(1)SpO2 changes:The group D at T4 and T5 were lower than group M and group C(P<0.05).The group C was higher than group M(P<0.05).Intra-group comparison:Compared with TO,the group M at T4 was lower(P<0.05).(2)RR changes:The group M at T4 and T5 were lower than group d and group C(P<0.05).Intra-group comparison:Compared with TO,the group M at T4 and T5 was lower(P<0.05).5.The PAED score and the incidence of emergence agitation in group D was lower than the other groups(P<0.05).6.The incidence rate of hypertension,hypertension and nausea in group D was lower than group C(P<0.05).The incidence rate of hypoxemia in group M was higher than group(P<0.05).Conclusion:Dexmedetomidine can be safetity and effectively used the pediatric laparoscopic indirect.It can reduce endotracheal intubation during stress reaction,stable hemodynamics and little effect on breathing.It has a good sedative and analgesia effect.lt yet can reduce postoperative agitation and the incidence of nausea and vomiting...
