BackgroundIn recent years,the research on mechanism of occurrence and development of coronary heart disease is still a big hot spot of clinical research.The abnormal state of blood coagulation and coronary atherosclerosis is a current research on mechanism of coronary heart disease theory in recently.Comprehensively to the real state of blood clotting in patients with right evaluation to the development of disease,disease prevention,disease diagnosis and treatment and prognosis of plays an important role in all aspects.Therefore,accurate efficient assessment of blood coagulation function in clinical laboratory,and master the blood coagulation state of coronary heart disease patients in time,was of great significance for the early prevention,diagnosis,and treatment of disease and prognosis.ObjectiveTo assess the coagulation state of coronary heart disease patients by thrombelastography(TEG)and conventional coagulation tests and to explore and compare the correlations and the superiority between the two methods in blood coagulation and platelet function testing.Method90 patients in Luoyang Center Hospital Department of Cardiology for treatment of PCI between January 2016 and December 2017 were collected in this study.All of the patients included in the study were based on clinical diagnosis on any function of liver andkidney damage,blood coagulation dysfunction,tumors,patients with active bleeding,severe infections,and other diseases.According to the characteristics of the patient and the specific clinical manifestations,it can be divided into acute myocardial infarction group(AMI group,n = 32),unstable angina group(UAP group,n = 29)and stable angina group(AP group,n = 29).Comparative analysis of three groups of patients with two kinds of detection method is applied in the correlation of blood coagulation and platelet function testing and superiority.With TEG method to detect the blood coagulation state variety of clotting index(R,K,MA,Angle,CI)and with the traditional coagulation experiment testing traditional coagulation indexes(PT,APTT,INR,Fbg,D-D,PLT).Results1.Patients with AMI and UAP groups show high coagulation state compare with the AP group.Thrombelastogram test parameters of the reaction time(R)and blood clot formation time(K)were significantly decrease,there was significantly statistical difference(P < 0.01).The parameter value of maximum amplitude(MA)and blood coagulation index(CI)and Angle(Angle)were significantly increased(P < 0.01).Conventional coagulation index test showed that compared with patients with AMI group,the other two patients tested specimens showed a significant rise in APTT and INR levels,there was significantly statistical difference(P < 0.01).Compared with a group of patients with AP,the other two groups of patients tested samples D-D and Fbg levels increased significantly(P < 0.01).Two kinds of detection methods for the evaluation of high coagulation state consistent.2.There has significant correlation between TEG related parameters and traditional coagulation index,embodied in the R and APTT,? and PLT,? and Fbg,MA and Fbg,MA and PLT,otherwise,the K value with the negative correlation of PLT and Fbg.3.Thrombelastogram for evaluating coronary heart disease patients with high blood coagulation state detection sensitivity significantly higher than that of traditionalcoagulation index.Compared with PLT,thrombelastogram alpha parameters and MA can better reflect the state of platelet function in patients with.ConclusionIn this study,there were high significant correlation between some paramter of thrombelastography and some of coagulation index of conventional coagulation tests;as an indicator and monitoring the state of blood coagulation in the patients with coronary heart disease,thrombelastography is useful.
