Background Non-alcoholic fatty liver disease(NAFLD)is one of the most common chronic liver diseases which is characterized by excessive accumulation of triglycerides in hepatocytes.The disease spectrum of NAFLD includes simple fatty liver,steatohepatitis(NASH),liver fibrosis,cirrhosis,and even liver cancer(HCC).The current study suggests that NAFLD is the hepatic correlate of the metabolic syndrome,a systemic disorder of energy homeostasis that often related to visceral adiposity,insulin resistance,oxidative stress,glucose intolerance,dyslipidemia.The incidence of NAFLD is increasing in our country.Epidemiological studies have shown that 15-20% adult have NAFLD.NAFLD has become the most common liver disease in China.As a world public health problem it should be get more attention.The occurrence and progress of NAFLD are closely related to genetic factors and environmental factors.The PNPLA3 rs738409 increase the risk of NAFLD and may increase triglyceride accumulation.The blood lipid is one of risk factors in coronary heart disease.Coronary heart disease and NAFLD often exist in the same patient.Coronary heart disease is one of the major causes of death in NAFLD patients.However,there is no study on the association between PNPLA3 rs738409 polymorphism and NAFLD patients with CAD.Objective In this study,we will investigate the association between rs738409 polymorphism of PNPLA3 gene and CAD in Chinese Han Population in China.And to explore the possible mechanism of NAFLD with CAD,.Methods The study enroll 257 NAFLD patients,include 144 simple NAFLD patients,113 NAFLD with CAD patients in Qingdao Municipal Hospital.Blood sample were obtained and collected for Genetic Analysis.Polymerase chain reaction analysis(PCR)and Matrix-Assisted Laser Desorption/Ionization Time-of-Flight Mass Spectrometry(MALDI-TOF MS)were used for genetic analysis of MBOAT7 rs641738.The distribution of genotypes and alleles was determined by sequencing.The Hardy-weinbeurg was used to evaluate the representative of the group.At the same time,the general data,clinical data and laboratory data were collected.SPSS22.0 was used to analyze the data of genotypes and clinical features by chi square test,t test and Logsitic regression analysis.Results We found a significant difference in genotype distribution in PNPLA3 rs738409 between the NAFLD patients with and without CAD(P<0.05).In the NAFLD with CAD,carriers of CC and CG show a significantly higher serum ALT,ALP,TG lever and lower HDL lever In the NAFLD without CAD,carriers of CC and CG show a higher BMI,ALT,AST,GGT,TG,TG and LDL lever.Conclusion In the Chinese Han population that we studied,NAFLD patients who carry the PNPLA3 G allele have an increased risk of developing CAD.
