Expression Of GATA3 In Upper Urinary Tract Urothelial Carcinoma And Its Clinical Significance

Objective:To investigate the expression of GATA3 in normal epithelium para-carcinoma tissue and carcinoma tissue of the upper urinary tract,and to explore its expression correlated with clinical characteristics.Methods: The clinical data and paraffin-embedded specimens of 73 patients with upper tract urothelial carcinoma who were treated by surgery at the Dalian medical university from Jan.2007 to Dec.2017 were collected.(Among them,55 cases were accompanied by para-carcinoma tissue of the upper urinary tract),24 patients with upper tract urothelial carcinoma underwent surgical treatment of normal distal urinary tract epithelial tissue paraffin-embedded specimens were be collected.The expression of GATA3 in tissues was detected by immunohistochemistry.The statistical methods were used to analyzed the relationship between the clinicopathologic features such as the age of diagnosis,sex,UICC T staging,the differentiation,the site,the side,the vascular tumor emboli of the tumor and so on,And to analyze the influencing factors of Disease-Free Survival(DFS)and Overall Survival(OS)in patients with upper tract urothelial carcinoma.Result:1.The positive rates of GATA3 normal epithelium,para-carcinoma tissue(100%)were significantly higher than that in the carcinoma tissue of the upper urinary tract(89%),the difference was statistically significant(P<0.05).The high expression rate(100%)was significantly higher in normal urothelial and upper urinary tract epithelial carcinoma tissues than in epithelial carcinoma of upper urinary tract(68.5%),thedifference was statistically significant(P<0.05).In upper tract urothelial carcinoma,the expression of GATA3 was significantly correlated with the pT stage of the patients(P<0.05),but there is no significant relationship between the clinicopathologic features,such as age,sex,the site,the side and the differentiation,the vascular tumor emboli of tumor,not statistically significant(P>0.05).2.For DFS,univariate analysis of all patients showed that pT stage and the differentiation of tumor were the factors that influencing postoperative DFS(P<0.05),there is no relationship between the expression of GATA3,age,sex,the site,the side and the vascular tumor emboli of tumor and postoperative DFS(P>0.05).As to muscle invasive upper tract urothelial carcinoma patients,univariate analysis showed that GATA3,the side of the tumor and pT stage were the factors affecting postoperative DFS of them(P<0.05),there is no relationship between sex,age,the site,and the vascular tumor emboli of tumor and postoperative DFS(P>0.05).Multivariate analysis showed that only the side of tumor was the independent prognostic factor of DFS of those muscle invasive upper tract urothelial carcinoma patients(P<0.05).3.For OS,univariate analysis of all patients showed that the differentiation of tumor was the factor influencing the OS of patients with upper tract urothelial carcinoma(P<0.05),the expression of GATA3,age,sex,the site and the vascular tumor emboli of tumor and postoperative OS(P>0.05).As to muscle invasive upper tract urothelial carcinoma patients,the results of univariate analysis showed that GATA3 was the major factor influencing OS in patients with upper tract urothelial carcinoma(P<0.05),sex,age,pT stage,the site,the side and the vascular tumor emboli of tumor and the postoperative OS of upper tract urothelial carcinoma was not significantly correlated(P>0.05).Conclusion:1.GATA3 has higher high expression in normal epithelium,para-carcinoma tissue and carcinoma tissue of the upper urinary tract,but its high expression rate in carcinoma tissue showed a decreasing trend.It might mean that,GATA3 may be involved in the occurrence of upper tract urothelial carcinoma.2.Among the upper tract urothelial carcinoma,the expression level of GATA3 was significantly correlated with T stage of tumor,It is suggested that GATA3 may participate in the infiltration of urothelial carcinoma.3.The high expression of GATA3 is a predictor of the good prognosis of epithelial cancer patients with muscle-infiltrated upper urinary tract.
