Objective: To investigate the predicting factors of the positive rate of transrectal ultrasound-guided biopsy for prostate cancer.Methods: Totally,560 patients who had a transrectal ultrasound(TRUS)-guided biopsy were retrospectively analyzed in the study from December 2013 to July 2016.The relevant data including age,total prostate specific antigen(TPSA),prostate volume(PV),body mass index(BMI),computed tomography(CT),and B-scan ultrasonography were analysed by univariate and multivariate regression and ROC.Results: 232(41.43%)cases were diagnosed with prostate cancer among the 560 patients.The positive rate of prostate cancers who aged <61,61-70,71-80 and above 80 years old was 17.19%,35.18%,52.31% and 62.50%,with significant difference between multiple groups(P<0.05).The positive rate of prostate cancers whose concentration of prostate specific antigen(PSA)ranged from <4,4-<10,10-<20,20-<40,40-<100 to ≥100ng/ml was 4.00%,24.70%,32.20%,55.30%,70.00%,98.33%,respectively with significant difference between multiple groups(P<0.05);the propotion of gleason score(8-10)was 0%,11.11%,14.29%,57.14%,57.14%,67.80%.The detection rate of prostatecancers with the prostate volume ranging from <25,25-<50,50-<80,80-<100,≥100ml was 77.14%,46.31%,30.72%,27.27% and 7.89%,with significant difference between multiple groups(P<0.05).The positive rate of prostate cancers with value of BMI ranging from <18.5,18.5-23.9,≥24 was 37.50%,42.96% and 40.08%,with no statistically significant difference between multiple groups(P=0.735>0.05).The positive rate of prostate cancers whose B-scan ultrasonography positive(Abnormal echo nodules)was 55.20% while negative was 37.35%,with significant difference between two groups(P<0.05);the CT positive(Abnormal enhancement)was 54.18% and negative was 38.04%,with significant difference between two groups(P<0.05).There were 41 cases(25.95%)whose f/TPSA<0.25 and 4 cases(16.67%)whose f/TPSA>0.25 when TPSA ranged from 4 to 10ng/ml,with no statistically significant difference between multiple groups(P>0.05).According to the multivariate Logistics regression analysis,it was found that the predicting factors of the detection rate of transrectal ultrasound-guided prostate cancers mainly included TPSA concentration,age,prostate volume and B-scan ultrasonography.By ROC,the diagnostic value of TPSA is the most and following is prostate volume,age and the least is B-scan ultrasonography,CT.Conclusion: Patients′ TPSA concentration,age,prostate volume and B-scan ultrasonography may predict the positive rate of transrectal ultrasound-guided prostate biopsy,analysis comprehensively to master a better indications of biopsy and timing,avoid excessive biopsy,help to improve the positive rate of prostate biopsy. |
