Objective:Through the observation of the patients,it was found that the syndrome of cold coagulation and blood stasis was the most among the patients with primary dysmenorrhea. In combination with the instructor’s clinical practice of many years, the hypothesis of“three steps of feeling cold"("the cold of heredity","the cold transmitted from the fetus","the cold of self feeling")was discussed. The essence, characteristics and clinical significance of symptoms were observed, and the analgesic effect of acupuncture was observed. Methods:In terms of theoretical research, based on the close relationship between the "three steps of cold sensation" and the occurrence of primary dysmenorrhea(cold coagulation and blood stasis syndrome),which summarize the clinical experience of the tutor for many years;in clinical research, among the primary dysmenorrhea patients with cold coagulation and blood stasis syndrome in the gynecology clinic and ward of the second affiliated Hospital of Heilongjiang University of traditional Chinese Medicine, which has 70 patients with cold coagulation and blood stasis syndrome were randomly selected and randomly divided into two groups according to the order in which they went to the doctor, which 35 cases in the acupuncture group, and 35 cases in the acupuncture group, which 35 patients in the control group were treated with ibuprofen sustained release capsule. The treatment of 3 menstrual cycles was a course of treatment, which the visual analogue scale(VAS)and dysmenorrhea symptom scores were observed in the menstrual period of the two groups after treatment, and the efficacy was determined 3 months after the end of the treatment. Results:The scores of VAS and dysmenorrhea symptoms in the two groups before and after treatment were statistically significant(P<0.05),which indicated that both acupuncture and ibuprofen could improve the degree of abdominal pain and reduce the incidence of dysmenorrhea symptoms. The VAS score and dysmenorrhea symptom scores were significantly different between the two groups(P<0.05)after treatment, indicating that the analgesic effect of acupuncture on primary dysmenorrhea was better than that of ibuprofen. After the end of treatment 3 months, which the scores of VAS and dysmenorrhea symptoms were compared between the two groups(P<0.05),indicating that the long-term efficacy of the acupuncture group was better than that of the ibuprofen group. The total effective rate of acupuncture group was 86.21%and that of ibuprofen group was 50.00%,which was statistically significant(P<0.05).Conclusion:1."Three steps of cold”can guide clinical and preventive care.2.Both acupuncture and ibuprofen can reduce the degree of abdominal pain during menstruation and relieve the accompanying symptoms, but the effect of acupuncture is better than that of ibuprofen.3.The long-term effect of acupuncture on primary dysmenorrhea is better than that of ibuprofen.
