| Objective:To explore the effect of the systematic nursing intervention of the doctor-nurse joint outpatient clinic on sleep quality of patients with peri-menopausal sleep disorders, and to provide reference for improving the process of out-patient medical and nursing service. Methods:The study adopted a generic experiment, according to the inclusion and exclusion criteria,we selected 136 patients with peri-menopausal sleep disorders in a department of gynaecologic outpatient department of a hospital in Changsha,Hunan,in January 2017 by means of simple random sampling method, which on the basis of visit serial number randomly were divided into the control group and the experimental group. The control group was given routine outpatient nursing intervention, and the experimental group was given the systematic nursing intervention of the doctor-nurse joint outpatient clinic. The baseline data were collected by the members of the research team who had been trained in a unified way, using questionnaires before the intervention of the two groups of patients. The data were collected by general information questionnaire, modified Kuperman index(mKI)and Pittsburgh sleep quality index(PSQI);what’s more, the modified Cupperman index(mKI)and Pittsburgh sleep quality index(PSQI)were used again 6 months after intervention. The scores of modified Kuperman index(mKI)and Pittsburgh sleep quality index(PSQI)before and 6 months after intervention were compared between the experimental group and the control group.SPSS23.0 software was used for statistical analysis after data collection. Results:Due to 8 cases of abscission, there were 2 cases in the experimental group and 6 cases in the control group. Finally, the effective sample size was obtained in 128 cases,66 cases in the experimental group and 62 cases in the control group.①Comparison of general data(age, height, weight, marital status, place of residence, level of education, work, monthly income, menstruation, personality, behavior, hobbies, weekly exercise, etc.),modified Kupperman score and Pittsburgh sleep index score of the two groups before, the differences were not statistically significant(P>0.05).②After 6 months of intervention, the mKI score of the experimental group decreased from 23.89±8.87 to 10.03±2.55(P<0.05)and decreased from 8.08 ±3.16 to 4.32 ±2.78,which was better than that before intervention. The difference was statistically significant(P<0.05).③After 6 months of intervention, the mKI score of the control group decreased from 24.53 ±9.59 to 15.29 ±5.39(P<0.05)and decreased from 8.15 ±3.97 to 6.45 ±2.76(P<0.05).The difference was statistically significant.④After 6 months of intervention, the mKI score of the control group was 15.29±5.39 and the mKI score of the experimental group was 10.03 ±2.55 and 4.32 ±2.78,respectively,which was better than that of the control group(P<0.05).Conclusions:(1)The systematic nursing intervention of the doctor-nurse joint outpatient clinic can significantly improve the sleep quality of the patients with peri-menopausal sleep disorder, improve the symptoms of peri-menopausal syndrome, and improve the quality of life.(2)The doctor-nurse joint outpatient clinic can provide personalized and specialized systematic nursing for the patients with peri-menopausal sleep disorder, which can improve the service flow of outpatient service and enhance the effect of outpatient medical care service. |
