BackgroundLumbar disc herniation is a spinal cord injury and degenerative disease that lasts longer and is one of the most common pain conditions affecting millions of people worldwide and may be a major cause of disability,depression and loss of job.With the rapid development of modern society and economy,the pressure from the middle-aged and young social groups also increases,LDH has also become a common disease in young adults.More and more evidence shows that,chronic pain injury inputs act on axons of different brain structures,which can cause changes in brain structure and function in different regions.Although many studies at home and abroad have described changes in brain function and structure in patients with chronic pain,changes in the cortical structure caused by patients with lumbar disc herniation have been reported with few changes.So far,there has been no consistent result of changes in brain structure due to chronic pain caused by lumbar disc herniation.ObjectiveTo study the changes of brain structure in patients with chronic lumbar disc herniation(LDH)based on voxel magnetic resonance morphometry.MethodsSelect the Second Affiliated Hospital of Xinxiang Medical College has been diagnosed with lumbar disc herniation and pain duration of more than 3 months 58 patients were case group(LDH group),Age 45-55;62 healthy volunteers matched with age and gender of case group were collected as normal control group(HC group),120 subjects were subjected to 3DT1 structural imaging using Siemens 3.0T MRI,The magnetic resonance data were analyzed by spm-VBM software.Finally,REST and AFN softwarewas used for statistical analysis.ResultsCompared with HC group,the volume of white matter(WM)in LDH patients was significantly different in the right cingulate gyrus,left frontal lobe,left superior temporal gyrus,Left inferior parietal lobule(P<0.05);In the right anterior lobe of cerebellum,right precentral gyrus,right posterior lobe of cerebellum,left fusiform gyrus,left superior frontal gyrus,right caudate nucleus,right middle frontal gyrus,left middle temporal gyrus,right cingulate areas of gray matter(GM)volume differences(P<0.05).ConclusionsThe use of VBM analysis method can accurately identify areas in which there are differences in gray matter and white matter in patients with LDH chronic pain.;At the same time,it can partially explain the cause of psychosomatic symptoms in patients,and has guiding value for the comprehensive clinical treatment of patients.At the same time,it can prompt promptly effective intervention measures for patients with ineffective conservative treatment.
