Objective:To monitor the depth of anesthesia through the Narcotrend anesthesia/EEG awareness system and to compare the changes in hemodynamics of tracheal intubation during induction of general anesthesia in elderly hypertensive patients at different anesthetic depths.We can conclude that the timing of tracheal intubation may be guided by the Narcotrend index.Methods:Sixty elderly patients with ASA grade II-III and hypertension were selected and divided into group A(E0:36-27)and group B(D2:46-37)and C groups(D1: 56-47),according to the Narcotrend Stage(NTS)during intubation.After admission,Narcotrend monitors were used to monitor depth of anesthesia.With Intravenous injection of sufentanil 0.3?g/kg and propofol intravenous infusion 6mg/kg/h,after the patient falls asleep,cis-atracurium benzenesulfonate 0.15mg/kg was injected.Until the NTS falls to a preset level.the trachea was intubated through mouth.Heart rate(HR),systolic blood pressure(SBP),diastolic pressure(DBP),Narcotrend index(NTI)of before anesthesia induction(T0),after anesthesia induction before tracheal intubation(T1),immediately after tracheal intubation(T2),1min after tracheal intubation(T3),3min after tracheal intubation(T4),5min after tracheal intubation(T5)are recorded,and incidence of adverse reactions such as physical activity,cough,intraoperative awareness are recorded,and then statistical analysis.Results:The systolic blood pressure,diastolic blood pressure,and heart rate of the three groups decreased at the time of after anesthesia(T1)compared with T0(p<0.05);the systolic pressure,diastolic pressure,and heart rate of the three groups increased immediately after the tracheal intubation(T2)compared with T0(p<0.05).Immediately after the tracheal intubation(T2),compared with groups B and C,systolic blood pressure was significantly lower in group A than in group B and C;The systolic blood pressure in group C was significantly higher than that in group A and B,and the difference was statistically significant(p<0.05).The number of cases of hypotension in group A was significantly higher than that of group B and C(p<0.05).The number of cases of hypertension in group C was significantly higher than that of groups A and B(p<0.05).Conclusion:When the Narcotrend index was D2: 46-37,the hemodynamics of tracheal intubation during general anesthesia in elderly hypertensive patients was relatively stable and the adverse reactions were less.It was an ideal depth of anesthesia.
