Objectives:To explore the effects of the integrated cardiac rehabilitation mode of doctor,nu rse,rehabilitation therapist and patient on clinical physiological index、self-managemen t behavior and quality of life after coronary stent implantation,to understand the curr ent situation of cooperation among doctors,nurses,rehabilitation therapist and patient in cardiac rehabilitation,to formulate the specific implementation method of the inte grated cardiac rehabilitation model,and to improve the self-management level of pati ents after coronary stenting.To promote early cardiac rehabilitation,to provide patient s with a more complete cardiac rehabilitation service security system,and ultimately to improve the quality of life of patients and prolong the life of patients.Method:From March to May 2017,120 patients diagnosed as coronary artery disease an d stenting by coronary angiography at the Cardiovascular Center of the First Hospital of Jilin University in Changchun City were selected as the study subjects.59 patien ts in April 2017 were selected as control group and 61 patients in May 2017 as exp erimental group.The patients in both groups were given nursing care according to th e nursing path of coronary heart disease.And received routine cardiac rehabilitation c are.On this basis,the experimental group was given the integrated cardiac rehabilitat ion mode of doctor,nurse,rehabilitation therapist and Patient,mainly taking holistic nursing as the guiding ideology,innovating the mode of nursing service and improvin g the flow of heart rehabilitation by applying the new rehabilitation concept.Self-ma de cardiac rehabilitation cases,allowing doctors,nurses,rehabilitation therapist,patients to form a seamless connection,to formulate targeted cardiac rehabilitation content.The intervention time was 6 months.Clinical physiological indicators(total cholesterol,triglyceride,high-density lipoprotein cholesterol,low-density lipoprotein cholesterol,b lood glucose,Hemoglobin A1 c,Blood pressure,BMI)、Coronary heart disease self-ma nagement behavior scale(CSMSM)and quality of life assessment(SF-36)were use d to evaluate the self-management,quality of life(QOLs)and physiological indexes of the two groups before,3 months and 6 months after intervention.To compare the difference of physiological index,self-management behavior and quality of life betwe en the two groups before and after intervention.Results:(1)Comparison of the baseline data: In the two groups,8 cases were lost in the experimental group and 4 cases in the control group.A total of 108 cases were followed up.There was no significant difference between the two groups in terms of demographic sociology and disease related characteristics(P>0.05),and the baseline data were comparable.(2)Comparison of physiological indexes: The experimental group was treated with the intervention of integrated cardiac rehabilitation mode of doctor,nurse,rehabi litation therapist and patient,Self comparison,all physiological indexes(except low d ensity lipoprotein and triglyceride BMI)were improved significantly(P<0.05).Compar ed with the control group,systolic blood pressure,diastolic blood pressure,total chol esterol,high density lipoprotein were significantly different from those in the control group(P<0.05),and after 6 months of intervention,there were significant differences between the two groups(P<0.05).The differences of physiological indexes(except l ow density lipoprotein)between the two groups were statistically significant(P<0.05).(3)Comparison of self-management behavior: The scores of self-management(except emotional management)were significantly improved in the experimental group after the intervention of integrated cardiac rehabilitation mode of doctor,nurse,reha bilitation therapist and patient,and the difference was statistically significan(P<0.05),compared with the control group.The scores of self-management behavior,daily life management and disease medicine management were higher than those of the control group(P<0.05).(4)Comparing the quality of life: the experimental group was treated with the intervention of integrated cardiac rehabilitation mode of doctor,nurse,rehabilitation therapist and patient,and its physiological function,physiological function,somatic p ain,vitality,emotional function,mental health,overall health and total score were sig nificantly improved.Compared with the control group,the scores of physiological fun ction,physiological function,somatic pain,vitality,social function,emotional function,mental health and overall health were significantly higher than those in the control group(P<0.05).Conclusion:(1)Integrated cardiac rehabilitation mode of doctor,nurse,rehabilitation therapi t and patient can help to improve blood pressure,blood sugar and blood lipid level,while low density lipoprotein,triglyceride and BMI are not significantly controlled.(2)After intervention,the patients in the experimental group got significant impr ovement except for the improvement of emotional management,which indicated that integrated cardiac rehabilitation mode of doctor,nurse,rehabilitation therapit and patie nt could promote the patients to correct their bad lifestyle and form good self-manag ement behavior.(3)Integrated cardiac rehabilitation mode of doctor,nurse,rehabilitation therapit and patient is helpful to improve the quality of life of patients,but the recovery of social function needs further study.
