Postoperative pain is a complex physiological response to the body caused by the disease itself and the surgical trauma.Research that confirms postoperative pain can have a very adverse effect on the patient.Good postoperative analgesia will be the goal of every medical worker.The aim of this study is to compare the analgesic effect of different doses of dexmedetomidine combined with sufentanil after abdominal hysterectomy,to determine a safer and more effective group for dexmedetomidin analgesia.In this study,80 patients undergoing elective abdominal hysterectomy under combined spinal epidural anesthesia in First Affiliated Hospital of Hebei North University from January 2017 to February 2018 were selected and agreed on the postoperative intravenous analgesia.The selected cases were randomly divided into four groups,which were group C,group D1,group D2,group D3 and 20 cases in each group.Each patient is connected to an intravenous analgesia pump at the end of the operation.Dosage of drug background in each group of analgesic pumps : Group C: sufentanil 0.04μg·(kg·h)-1;group D1: sufentanil 0.04 μg·(kg·h)-1 + dexmedetomidine 0.03 μg·(kg·h)-1;D2 group: sufentanil 0.04μg·(kg·h)-1 + dexmedetomidine 0.06μg·(kg·h)-1 and D3 group: sufentanil 0.04 μg·(kg·h)-1 + dexmedetomidine 0.09μg·(kg·h)-1,combined with normal saline to 100 ml,the injection rate of the analgesic was 2ml·h-1.The analgesic score(resting VAS score)and sedation score(Ramsay sedation score),satisfaction,blood pressure,heart rate and adverse reactions were recorded at four time points after 6h,12 h,24h and 48 h.The results show that: 1.There were no significant differences between the four groups in the general and intraoperative conditions(P>0.05).2.The VAS scores and Ramsay scores of the four groups at different time points were the highest in C group,and the rest from low to high were D3,D2 and D1 groups,and the difference was statistically significant(P<0.05).3.Postoperative satisfaction scores were compared,the scores of the C group were the lowest,and the scores of the three groups were significantly higher(P<0.05),and there was no significant difference in the D2 and D3 groups(P>0.05).4.Blood pressure(systolic blood pressure,diastolic blood pressure)and heart rate of each group at different time points were lower than those of group C,and the difference was statistically significant(P<0.05).5.The incidence of adverse reaction was higher in group C after operation,and the other three groups were significantly lower than those in the control group(P<0.05).The results of the experiment are as follows:1.Dexmedetomidin is effective for intravenous analgesia after abdominal hysterectomy.2.Sufentanil 0.04 μg·(kg·h)-1 + dexmedetomidin 0.06 μg·(kg·h)-1 used for analgesic sedative effect is significant and safe.
