ObjectivePart 1:To analysis the clinical characteristics of patients with primary liver cancer and the effects of blocking blood on liver function during the surgical,and hope to provide some reference for the early diagnosis,prevention and election of surgical procedure of primary liver cancer.Part 2:To explore the correlation of pathological types of primary liver cancer and HBV infection patterns and the content of AFP,and hope to provide new references for evaluating the prognosis of primary liver carcinoma.Part 3:To explore the effect of HBV infection patterns on the prognosis and survival duration of primary liver cancer patients after hepatectomy.MethodsPart 1:To retrospective analysis the data of patients with primary liver cancer,who received hepatectomy at the First Affiliated Hospital of Sun Yat-Sen University,Nanfang Hospital of Southern Medical University and Shunde Second People’s Hospital from January 2003 to December 2013.It contained age,gender,source,clinical symptoms,past medical history,family history,tumor size,tumor number,tumor site,whether vascular invasion,whether to block blooding during hepatectomy,the liver function of patients before and after operation and so on.Part 2:The clinical data of 1126 patients with primary liver cancer who received hepatectomy at the First Affiliated Hospital of Sun Yat-Sen University,Nanfang Hospital of Southern Medical University and Shunde Second People’s Hospital from January 2003 to December 2013 were retrospectively analyzed.All patients were divided into hepatocellular carcinoma group,cholangiocarcinoma group and other types of liver cancer group,then to analyze the correlation between the 3 pathological types and HBV infection patterns and the content of AFP.Part 3:A retrospective analysis of the clinical date and follow-up dates of primary liver cancer patients after hepatectomy at the First Affiliated Hospital of Sun Yat-Sen University,Nanfang Hospital of Southern Medical University and Shunde Second People’s Hospital from January 2003 to December 2013,and then to explore the effect of HBV infection patterns on the prognosis and survival duration of primary liver cancer patients after hepatectomy by COX.ResultsPart 1:1)86.9%of primary liver carcinoma patients were males,13.1%were females and in Guangdong,with an average age of 49.74±12.41 years.The proportion of patients from the pearl river delta was the highest,at 48.5%.2)PLC patients showed a variety of clinical manifestations,accounting for 42.4%of all patients with abdominal pain,and 35.7%patients had no manifestations and were found PLC during physical examination.3)83.4%PLC patients showed HBsAg(+),and 15.8%of patients had a family history of liver carcinoma.4)In this study,73.6%patients had only one tumor,51.5%were lump type liver cancer,and 75.9%of the envelope of the tumor were complete.5)The serum levels of indirect bilirubin was effected if blooding during hepatectomy(P<0.05)Part 2:1)No obvious correlation between age,gender and pathological types of primary liver cancer existed.2)There were significant differences in serum AFP levels in patients with different pathological types of primary liver cancer,and the serum AFP levels in patients with hepatocellular carcinoma were significantly higher than those of other pathological types.3)Obvious correlations between HBV infection patterns and pathological types of primary liver cancer were found,the positive of HBsAg+HBcAb+HBeAb was highly associated with hepatocellular carcinoma,the positive of HBsAg+HBeAg+HBcAb was associated with hepatocellular carcinoma and the positive of HBsAb was associated with cholangiocarcinoma group to some extent.Part 3:1)The survival time had on statistically significant differences in different age,gender in patients with PLC.2)The survival time was significantly different in PLC patients with different HBV infection patterns,and the patients with the pattern of the positive of HBsAg+HBeAg+HBcAb had the highest risk of death.ConclusionThe majority of PLC patients were males,and mainly came from the pearl river delta region in guangdong province.The serum levels of indirect bilirubin was effected if blooding during hepatectomy.Pathological types of primary liver cancer was associated with HBV infection patterns and the content of AFP,and the patients with different HBV infection patterns has different survival time.Therefore doctors should pay more attention to HBV infection.
