| Objective Perimenopause is a typical transition stage in a woman’s whole life,women in this stage always troubled by insomnia,which imposed negative effect on their physical and mental health.To observe the changes of regional brain activity in resting-state perimenopause insomnia by using amplitude of low frequency fluctuation(ALFF)and regional homogeneity(ReHo)methods,and to investigate the correlation between abnormal brain area in ALFF and ReHo and Pittsburgh sleep quality index(PSQI)in perimenopause insomnia.Methods 20 cases of perimenopause insomnia patients and 20 perimenopause normal sleepers matched in age and education level were selected as volunteers.Two groups all had Pittsburgh sleep quality index,Hamilton depression scale and Hamilton anxiety scale by the same psychiatrist.Using the America GE Discovery 750w 3.0T superconducting MRI scanner and 32 channel phase front head circle for regular whole brain scan,three-dimensional brain structure scan and resting-state functional magnetic resonance imaging(rs-fMRI)scan.Pre-processing the rs-fMRI data and analysis data by ALFF,ReHo methods in DPARSF 4.3 software.Independent sample T-test between the general data of two groups were performed by SPSS 17.0.All factor variance analysis(ANOVA)between rs-flvRI(ALFF,ReHo)data of two groups,and correlation analysis between changed brain regions in rs-fMRI results of perimenopause insomnia with Pittsburgh sleep quality index were performed by REST software(age and education level as the convariant).Results 1.ALFF analysis,Compared with perimenopause normal sleepers,perimenopause insomia group showed increased ALFF in left thalamus and splenium of corpus callosum.The ALFF value dfferences of the two groups was showed in the statistical parametric mapping by AlphaSim correction(the threshold was set at P<0.05,cluster size≥45).2.ReHo analysis,Compared with perimenopause normal sleepers,perimenopause insomia groupincreased ReHo in right frontal lobe,especially superior frontal gyrus and middle frontal gyrus.The ReHo value dfferences of the two groups was showed in the statistical parametric mapping by AlphaSim correction(the threshold was set at P<0.05,cluster size≥43).3.No correlations between these abnormal ALFF,ReHo brain regions and Pittsburgh sleep quality index(P>0.01).Conclusion 1.Perimenopause insomia with ALFF,ReHo abnormal changes in left thalamus,splenium of corpus callosum and right frontal lobe,suggesting that the corresponding brain area of neuronal activity intensity as well as homogeneity have changed,indicating insomnia symptom of perimenopause women can lead to abnormal brain function in the corresponding area.Left thalamus activated the right frontal lobe through splenium of corpus callosum,and then affected sleep-awake cycle of perimenopause women.2.Combined using rs-fMRI two analysis methods(ALFF,ReHo)can effectively evaluate the resting state of brain function changes,it became more conductive to provide functional neuroanatomic foundation to explore possible neurological pathophysiological mechanism and potential treatment project. |
