| Objective: To explore the practical and potential impact of classified diagnosis and treatment on the economic operation of public hospitals,and to put forward some suggestions on the implementation of the classification system and the direction of the development of public hospitals.Methods: The methods of combining the qualitative and quantitative methods such as literature research,expert consultation,statistical analysis,comparative study,case study and prospective study are adopted.On the one hand,the allocation of medical resources,the supply and demand of medical services,the change of medical services,the adjustment of the public medical hospital and the public medical treatment,and the public medicine are on the two aspects of the macro and micro aspects.The efficiency and efficiency of hospital operation on the operation of the national medical service system,the economic operation of public hospitals in three pilot cities of Xiamen,Ningbo and Qingdao were studied and analyzed comprehensively.On the other hand,the hypertension disease was selected as the research disease,according to the different levels of hypertension.The hospitalization standard of different hospitals was used to analyze the hypertensive patients in different hospitals,to judge the proportion of the standard of diagnosis and treatment,to analyze the related expenses of hospitalization,and to study the economic impact of the classification and classification of hypertension on the current public hospitals.Results: In the background of the national implementation of the system of grading diagnosis and treatment,the increase rate of emergency person times and the increase rate of discharge number of hospital in three cities of Xiamen,Ningbo and Qingdao were decreased in varying degrees.The degree of decline was positively related to the time of implementing the classification system and the intensity of policy.Among them,the number of outpatient and emergency visits in Ningbo’s two grade hospitals in 2015 and the number of outpatients and emergency visits in the three level hospitals in Xiamen and the number of discharged hospitals in two or three hospitals all showed negative growth.Through the case study of the pilot work of the slow disease management in Qingdao and the prospective study of the classified diagnosis and treatment of hypertension,it is found that the implementation of the classification based on the clinical path and the disease and the implementation of the ideal model can guide the patients to choose the suitable public hospitals,reduce the cost and burden of the patients,and the hospitalization of the public hospitals.Volume and hospital income will be reduced,and economic operation will be greatly affected.Conclusion: The grading diagnosis and treatment can guide the patients’ shunting,reduce the cost and burden of the patients,control the medical service of the public hospitals,and then affect the income and expenditure of the public hospitals.Through the research and analysis,the classification diagnosis and treatment system is the key link to promote the transformation and development of public hospitals and promote the reform of the supply side of public hospitals and implement the classification system based on the clinical path and disease classification management.It can further improve the rational allocation of medical and health resources and optimize the medical service.Supply,clarify the development orientation of medical institutions,improve the health literacy and health level of residents,improve residents’ sense of medical treatment,promote the smooth implementation of China’s medical supply side reform,and build a scientific and orderly new mechanism of medical and health operation for promoting the construction of "Healthy China". |
