Self-comparison Between Patella Synovium Transplantation And Patella Replacement In Bilateral Total Knee Arthroplasty At The Same Time

Objective: Compared to the clinical effect of total knee arthroplasty(TKA)on the treatment of knee degenerative osteoarthritis(OA),patellar synovial graft and patella replacement.Methods: According to the American rheumatism association(American society for rheumatology)of inclusion criteria and exclusion standard strictly,the research object to choose since December 2015 to November 2016,due to the double knee degenerative osteoarthritis(OA)in Qingdao university medical college affiliated hospital huangdao campus admitted,to prepare the same line of bilateral total knee replacement in patients with 55 cases(110 knees).Based on computer generated random number table,55patients(110 knees)were randomly divided into synovial package and patella replacement group,including synovial package group is 55 knees(22 knees one on the left,and right knee for 23 cases)in total knee replacement for patellar line synovial package.In addition,the patella replacement group was also 55 knees(with 24 knees on the left and 21 in the right knee)and patella replacement.Patients in the study using the same knee prosthesis(all implants used Micro Port companies with the Advance of inner shaft surface of total knee replacement system),are performed for the same experienced doctors work completed.Detailed observations synovial package with patella replacement group of preoperative and postoperative 3,6,9 and 12 months of KSS score,patellar Feller score,activity degree of knee joint,and the incidence of knee pain patients to the satisfaction of the operation indicators,based on the preoperative and postoperative X-ray of synovial package and patella replacement group imaging evaluation.Results: The KSS functional score and the activity degree of knee joint were significantly improved after the operation of the patella synovial and patella replacement group.There were no statistically significant differences in KSS clinical scores between the patients with synovial inclusion group and the patella replacement group at 3,6,9and 12 months after surgery(P > 0.05).Two groups of patients for 3 months and 6months after surgery,in the aspect of KSS function score no statistical difference(P >0.05),but after 9 months,12 months of KSS score difference was statistically significant,synovial parcel score higher than the patellar replacement group(P < 0.05).After a follow-up study,synovial package with patella replacement group 3 months after surgery,patients with Mr.Feller score no statistically significant differences(P > 0.05),but the synovial package group of patients with patellar replacement patients postoperative 6,9and 12 months in patellar Feller score difference was statistically significant,synovial package is better than that of patella replacement group(P < 0.05).Synovial package group of patients with patellar replacement group of patients after TKA surgery are to a certain extent,reduce the incidence of knee pain,two groups of 6,9 and 12 months after surgery in patients with the incidence of knee pain before the difference was statistically significant,patella replacement knee pain incidence is higher than before the synovial package group(P < 0.05).In postoperative imaging,there was no statistical difference between the synovial package group and the patella replacement group(P > 0.05).Conclusion: In total knee arthroplasty,the application of patellar synovial wrapping can significantly improve the knee joint function of patients after total knee arthroplasty.In boc patellar synovial packages,total knee arthroplasty can help reduce the incidence of knee pain before and after knee arthroplasty is more conducive to functional recovery of patients after total knee arthroplasty,knee joint activities.
