| Objective to compare the effects of percutaneous approach and transmuscular approach on thoracolumbar fractures.Methods a total of 78 patients with thoracolumbar fractures were treated in our hospital from February 2014 to November 2017.The patients were randomly divided into 2 groups(experimental group and reference group),including 40 patients in the experimental group and 38 cases in the reference group.The Wiltse muscle gap approach was used in the experimental group,and the minimally invasive approach was adopted in the reference group.The two groups of surgical incision length,intraoperative bleeding volume,postoperative flow rate,operation time,intraoperative fluoroscopy,pain score,and 12 months after the operation were compared with the T2 signal rate,and the patient serum was collected and the muscle enzyme was detected.The results of this study were: the difference of the length of incision,the amount of bleeding,the flow rate and the time of hospitalization in the two groups was not significant(P > 0.05).The operation time of the experimental group was significantly shorter than that of the reference group,and the difference was significant(P < 0.05);the patients in the experimental group were in the operation.The frequency of fluoroscopy was significantly shorter than that of the reference group,with significant difference in data(P < 0.05).There was no significant difference in the content of Myo,LDH and CK in the serum of the two groups before the operation(P > 0.05),and the content of Myo,LDH and CK in the serum of the patients in the experimental group was lower than that of the reference group after 3 days of operation.The difference of data was significant(P< 0.05);the 12 months after operation two groups of two groups were compared with T2 signal rate,and the difference was significant(P < 0.05).Conclusion: Wiltse intermuscular approach and percutaneous approach for asymptomatic thoracolumbar fractures are the main surgical approaches.The Wiltse muscle gap approach has the advantages of short fluoroscopy time,short operation time,short operative time,easy to master and relatively low equipment requirements. |
