| In January 2016,the State Council issued the opinions on integrating the basic medical insurance system for urban and rural residents.It put forward the integration of urban residents’ basic medical insurance and the new rural cooperative medical system,and set up a unified basic medical insurance system for urban and rural residents,that is,"two guarantees combined with one".The integration of urban and rural residents’ basic medical insurance system can make citizens enjoy equal rights,allocate medical resources more effectively,and enhance the ability of medical insurance fund to resist risks.Although studies have shown that the transformation from low-grade medical insurance to high-grade health insurance can improve the health level of residents,promote the utilization of medical services,and improve the actual reimbursement rate of patients,but its impact on the medical burden of patients is not clear.What is the effect of "two guarantees" on medical expenses of urban and rural residents? Can it solve the problem of the rapid growth of medical payment under the original NCMS system and the failure to decrease the self-payment of the insured personnel? Under the background of the new medical system,how to improve the medical welfare of urban and rural residents and alleviate the problem of "difficult to see a doctor" and "see a doctor" ? The research on the medical burden of residents under the background of "two guarantees and one in one" is closely related to the smooth operation of the system and the direction of China’s medical system reform.It has important theoretical and practical significance.Combined "two guarantees" system background,this paper takes G city as an example introducing the integration of urban and rural residents health care practices and cost control measures to preliminary analysis of changes in medical expenses N area after integration,then we use the breakpoint regression to analysis the effect of rural and urban residents medical insurance on medical expenditure for urban and rural residents and then test reliability of the conclusion of the changes of the relevant expenses.The study found that after the integration of basic medical insurance for medical expenses of urban and rural residents in N area of steady growth,but the growth rate slowed down in the hospitalization expenses and cost of outpatient growth speed.The regression evidence shows that urban and rural residents in health care integration and change characteristics of these costs has significant causal relationship.Combined with the research conclusions of medical insurance cost control of urban and rural residents in N District,this paper draws on the successful experience and practice of foreign medical insurance cost control and finally put forward relevant countermeasures and suggestions,and actively respond to the new situation of current medical expenses change. |
