| ObjectiveTo evaluate the effect of the recombinant human erythropoietin on perioperative demand of allogenic transfusion in patients undergoing orthodontic and orthognathic combined operation.MethodsSixty patients undergoing orthodontic and orthognathic combined operation were randomized divided into control group and experimental group,thirty patients in each group.For the experimental group,5000 u of recombinant human erythropoietin was injected subcutaneously at seven day(T1)and one days(T1)before surgery,and 100 mg of iron sucrose was injected intravenously at one day before surgery(T2).The control group did not receive rHuEPO and iron sucrose,and the other treatments were the same as in the experimental group.Reticulocyte percentage(RET%),hematocrit(HCT),hemoglobin(Hb),red blood count(RBC),platelet count(PLT)were respectively detected and recorded at seven days(T1)before surgery,one day before surgery(T2),one day(T3)after surgery,three days(T4)after surgery,seven days(T5)after surgery and 30 days(T6)after surgery in two groups.Glutamic pyruvic transaminases(GPT),aspartate transaminase(AST),blood urea nitrogen(BUN),serum creatinine(SCr)were respectively recorded in experimental group before and after rHuEPO therapy.Operation time,perioperative blood loss(include intraoperative and postoperative blood loss),allogeneic blood transfusion and adverse reactions were respectively recorded in two groups.ResultsRET%,HCT,Hb,RBC,and PLT were no significant differences between two groups at T1(P>0.05).Compared with the control group,the value of RET%,HCT,Hb,RBC in experimental group were higher than those in control group at T2,T3,T4,T5(P<0.05).Hb level in the experimental group returned to normal at the T5 time point,while Hb level in the control group was still significantly lower than the preoperative basic level(P<0.05).There was no obvious difference of PLT count among the two groups at the same period(P >0.05).There was no significant difference of GPT,AST,BUN,SCr in experiment group before andafter the operation(P>0.05).No significant differences were noted between the two groups in regard to perioperative blood loss(P >0.05),while allogeneic blood transfusion in experimental group was less than control group(P <0.05).There was no complications such as thrombosis in both groups during follow-up.ConclusionPreoperative erythropoietin in orthodontic and orthognathic combined operation can effectively raise hemoglobin levels and reduce blood transfusion.There were no significant effects on the renal or hepatic function and no increase the incidence of complication such as thrombosis. |
