Objective: to study the magnetic resonance Diffusion Tensor Imaging(coursing together Tensor Imaging,DTI)in intracranial diagnosis and differential diagnosis of benign and malignant tumor,malignant tumor infiltration degree and act on the clinical application value of tumor surgery.Materials and methods:(1)collected from 2015 to 2017,shihezi university school of medicine in the first affiliated hospital of 47 patients with intracranial space-occupying lesions admitted in neurosurgery,19 cases of male,female 28 cases,aged 20 to 77 years old,average age(52.51±10.73),all patients were performed routine MRI and DTI scan sequence.DTI sequence acquisition is completed,through the image post-processing,getting FA(Fractional Anisotropy of the anisotropic)figure,ADC(Apparent coursing together Coefficient and Apparent Diffusion Coefficient)figure,the FA value and ADC value of the tumor parenchyma region,tumor peripheral region and normal white matter area were measured,and the value of r FA(Relative FA,Relative anisotropic)and r ADC(Relative dispersion coefficient)were calculated.The difference of FA value,ADC value,r FA value and r ADC value of three different tumor types were analyzed;the statistical differences of FA value,ADC value,r FA value and r ADC value were studied in the tumor parenchyma of benign and malignant tumors;the statistical differences between the FA value,ADC value,r FA value and r ADC value of normal white matter area were analyzed.SPSS 20.0 statistical software was used for statistical analysis.In this paper,the difference between the three groups was compared by one-way anova,and the lsd-t Test was used to compare the differences between the three groups.T test of two independent sample parameters was used to analyze the difference of each index of benign and malignant tumor.A paired sample t test was used to compare the differences in the indexes between the white matter area and the normal white matter area.P < 0.05 indicates statistical significance.(2)30patients were selected as the experimental group on the intracranial screen,19 cases of male,female in 11 cases,aged 31-73,the average(59.5 ± 9.28)years of age,all patients underwent routine MRI scan,enhanced scan and DTI sequence scan,and the neural navigation technique was used in the operation,which was combined with magnetic resonance data to perform tumor resection;from January 2012 to December 2017,a total of 28 patients with intracranial infarction admitted by neurosurgery in our hospital were retrospectively analyzed as control group,the male 15 cases,13 cases of female,aged 18-61,the average(44.3±6.92)years of age,preoperative MRI scan and conventional enhanced scan sequence check,in the traditional way of surgery for tumor resection.SPSS 20.0 statistical software was used for statistical analysis.Two tests were performed using the paired four-grid data,the differences between the tumor excision rate,postoperative disability rate and postoperative new motor dysfunction were compared between the experimental group and the control group.P < 0.05 indicates statistical significance.Results:(1)This group of 47 cases of patients with intracranial placeholder tumor types were confirmed by pathological results after surgery: 22 cases with meningioma(both for the WHO Ⅰ level),16 cases of gliomas(both for high grade glioma),metastatic tumor 9 cases.Comparison between three kinds of tumor,tumor nature area FA value: meningioma(0.20±0.11)> glioma(0.12±0.05)> metastases(0.10±0.05),at the same time,comparison between any two,tumor parenchyma area FA value in meningioma and glioma,meningioma and comparing differences between metastatic tumor was statistically significant(P<0.05);There was no statistically significant difference between glioma and metastatic tumor(P>0.05).Tumor parenchyma area r FA value: meningioma(1.08±0.75)> metastases(0.60±0.35)> glioma(0.49±0.34),at the same time,comparison between any two,tumor parenchyma area FA value in meningioma and glioma,meningioma and comparing differences between metastatic tumor was statistically significant(P<0.05);There was no statistically significant difference between glioma and metastatic tumor(P>0.05).Tumor parenchyma area ADC values: meningioma(6.87±2.32)> glioma(2.88±2.92),and the comparison between difference was statistically significant(P<0.05),meningioma and metastases,glioma and essence area ADC value comparison between metastatic tumor,there were no statistically significant difference(P>0.05).There was no statistically significant difference between the r ADC values in the tumor parenchyma.Comparison of benign and malignant tumors: the FA value of tumor parenchyma: benign tumor(0.20±0.11)> malignant tumor(0.11±0.05),and the comparison between the two was statistically significant(P=0.01).RFA value of tumor parenchyma: benign tumor(1.08±0.75)> malignant tumor(0.53±0.34),and there was a statistical difference between the two(P=0.03).ADC value of tumor parenchyma: benign tumor(6.87±2.32)> malignant tumor(2.88±2.92),and there was a statistical difference between the two(P=0.01).There was no statistically significant difference between the tumor parenchyma(1.68±1.84)and > malignant tumor(0.94±1.72).There was no statistically significant difference between the normal white matter area(0.34±0.11)and the white matter area(0.25±0.13)in the normal white matter area(0.34±0.11).r FA value: there was a statistically significant difference between the normal white matter area(1.03±0.37)> tumor week white matter area(0.35±0.13),and the difference between the two was statistically significant(P=0.000).ADC value: there was no statistically significant difference between the normal white matter area(7.79±1.47)> tumor week white matter area(6.75±3.10).r ADC value:(7.79±0.69)> on the normal white matter area(1.01±0.21),the difference between the two was statistically significant(P=0.000).(2)the total cutting rate of tumor in the control group was 50.00%lower than that in the experimental group 83.33%(P = 0.007).The disability rate of the control group was35.71% higher than that of the experimental group 6.67%(P = 0.006).The difference between the two groups was statistically significant.In the two groups of patients,the new status was found to be aphasia.In the control group,the rate of postoperative aphasia was 14.29 percent after surgery,and the rate of aphasia was 6.67%(P >0.05).There was no statistically significant difference between the two groups.Conclusion: 1.The FA value,r FA value and ADC value of DTI are significant in the diagnosis of brain tumor type.2.The FA value,r FA value and r ADC value of DTI multi-parameter values are of great value in determining the peri-infiltration range of malignant tumor.3.DTI in guiding supratentorial intracranial tumor patients have played an important value in the operation,can the minimum effective pyramidal tract injury,resection of brain tumors to the greatest extent,improved the tumor resection rate and survival quality.
