Correlation Between Luteinizing Hormone Level And TCM Constitution,Syndromes Type And Influencing Factors In 147 Cases Patients With PCOS

Objective:To investigate the influencing factors and characteristics of luteinizing hormone levels with Polycystic Ovary Syndrome(PCOS),and to explore the value of luteinizing hormone in the diagnosis of polycystic ovary syndrome(PCOS).To understand the relationship between LH / FSH and TCM constitution and TCM syndromes,and To provide new basis and new ideas for the prevention and treatment of this disease.Methods: First of all,the diagnostic criteria of the case group and the inclusion and exclusion criteria of the healthy control group were determined.The patients who met the inclusion criteria were investigated by questionnaire,the TCM syndromes were determined by two deputy directors or one chief physician according to the standard of TCM syndrome differentiation,and the TCM physique was judged by referring to "Classification and judgment of TCM physique",and the results of the investigation were entered into the Excel table.the results of the investigation were entered into the Excel table,and the data were analyzed statistically by SPSS21.0.Result:1.A total of 147 patients with PCOS were investigated.The values were higher than those in the normal control group.The level of LH in PCOS group was higher than that in PCOS group over 40 years old,and the difference was statistically significant(P<0.05).2.There were 19 cases(12.93%)with low weight in 147 patients with POCS,94cases(63.9%)with normal body weight and 34 cases(23.1%)with overweight.In the healthy control group,5 cases(12.5%)were underweight,31 cases(77.5%)were normal weight,and 4 cases(10%)were overweight in PCOS group.The body mass index(BMI)of the two groups was statistically significant(P < 0.05),and the LH/FSH decreased with the increase of body mass index(BMI)in the BMI group.There was statistical significance(P < 0.05).3.There were 61 cases(41.4%)of PCOS with hyperandrogenemia.The ratio of LH / FSH in patients with PCOS was higher than that in normal controls,and the ratio of LH / FSH in PCOS patients was positively correlated with androgen.The difference was statistically significant(P < 0.05).4.There were 65 cases(44.2%)with insulin resistance in 147 patients with PCOS,and the LH level in the group with or without insulin resistance did not change with the increase of insulin resistance index.The ratio of LH / FSH decreased with the increase of insulin resistance index.But there was no statistical significance(P >0.05).5.There was no significant difference in LH levels between infertile and fertile PCOS patients(P > 0.05).5.There was no significant difference in LH level in 147 PCOS patients with or without history of infertility(P > 0.05).6.The 75 th、95th percentile of LH in the normal control group was 5.6U / L 、6.6U / L,and in PCOS group was 128,112 cases,respectively,the number of the75th、95th percentile of the normal control group was 1.3,1.8,and that of the normal control group was 128,112 cases respectively.When LH/FSH≥2,the area under the curve is 0.86,the sensitivity is 0.73 and the specificity is 0.99.When LH/FSH≥2,the diagnosis has higher specificity and accuracy.7.The order of LH mean of 147 patients with PCOS from high to low was:dampness and heat > yang deficiency > qi stagnation > calmness > blood stasis >phlegm and dampness > yin deficiency > qi deficiency.The serum LH level of damp-heat and yang deficiency was higher than that of other physical groups.The difference of dampness and heat,yang deficiency and qi deficiency was statistically significant(p < 0.05),and the LH/FSH ratio of yang deficiency was higher than that of other physical groups,but there was no significant difference(P > 0.05).8.The common TCM syndromes of PCOS patients are as follows: kidney yang deficiency and liver depression,kidney yang deficiency and blood stasis,kidney yang deficiency,kidney yin deficiency phlegm dampness,among which compoundsyndrome type,deficiency and solid inclusion is the main type,and document type is kidney yang deficiency in turn.Liver depression,phlegm and dampness,in which kidney yang deficiency LH level was significantly higher than other groups.Conclusion :1.The LH level and LH / FSH ratio of PCOS patients were affected by androgen,obesity and other factors,which were positively correlated with androgen,negatively correlated with body weight,and correlated with age.2.Insulin resistance is not associated with the increase of LH and LH / FSH ratio,which is an inherent endocrine feature in patients with PCOS.The increase of LH level and the ratio of LH / FSH may be included in the diagnostic criteria of our country.When LH/FSH≥2,the diagnosis has higher specificity and accuracy.3.This survey of PCOS patients with gentleness constitution,damp heat,Yang deficiency,damp heat,serum LH levels were higher than other matter Yang,LH/FSH physical group;syndrome types PCOS complex syndrome,deficiency,the deficiency of kidney yang,and stagnation of liver qi,phlegm,blood stasis,kidney yang LH level was significantly higher than other syndromes.
