Objective: Anorectal dynamic discipline is a new subject that researching in the movement of colorectal and anal canal(including pelvic floor),which plays an significant role in the process of diagnosis and treatment of constipation.Unrelaxed pelvic floor syndrome(UPFS),as one kind of the chronic functional constipation,is closely related with the abnormal changes of anorectal dynamics.This study is based on the high temperature water enema theory in the treatment of UPFS,and the randomized,single-blind,controlled clinical researches are used to evaluate anal resting pressure,rectal resting pressure,rectal maximum systolic blood pressure,rectal force exhaust pressure,differential pressure of anal exhaust pressure,rectal,initial sensory threshold,continuous meaning is capacity,maximum volume,atony defecation reflex,comfort and complications in patients with rectal compliance after the treatment of high temperature water enema.It can provide a reference for the future high temperature water enema technology,reduce the patient’s pain and inconvenience,and improve the quality of life of patients.Methods: 120 cases of UPFS,obtained from Chengdu anorectal specialist hospital and Sichuan intercontinental gastrointestinal disease hospital,were randomly divided into the high temperature water enema group(the test group)and the traditional conventional enema group(the control group),both of them are 60 cases.The Prospective clinical study are carried out in this research,during which the indexes of the enema process of two groups(enema number,enema back row number,total retention time,every time the sum total of enema retention time,intestinal juice combined liquid volume,filling and intestinal juice spillover,heart palpitations,abdominal pain,hematochezia,mucous membrane)are recorded.While the indexes for evaluation before and after the enema process(anal canal resting pressure,rectal resting pressure,rectal maximum systolic blood pressure,exhaust pressure,anal canal rectum force force exhaust pressure,rectal pressure,initial sensory threshold,continuous meaning is capacity,maximum volume,atony defecation reflex,rectal compliance)are also measured.Then,SPSS21.0 statistical software was used to analysis the results.If P>0.05,there is no statistically significant difference,if P<0.05,the difference was statistically significant,but if P<0.01,there was a significant statistical significance difference.Results: 1,as for the observation results of average ARP of UPFS patients,the experimental group is lower than the control group with type I(P<0.01),but the results show that the type II,III,IV had no significant difference(P>0.05);Average RRP after enema in patients with type II,IV of experimental group is higher than that of control group(P<0.05);Type I and III after enema differences between the two groups have no statistical significant difference(P>0.05);The average of MSP of two groups UPFS patients with four types(I,II,III,IV)had no statistical significant difference(P>0.05).2,The average pressure of rectal defecation after enema of experimental group is higher than that of control group(P<0.01);The average force of anal canal pressure after enema between the two groups have no significance difference(P>0.05).The average rectal pressure of experimental group is lower than that of control group(P<0.05).3,As for the average FSV,the results of UPFS patients with type I and II in experimental group were lower than that in control group(P<0.05),while patients with type III and IV had no statistical significant difference(P>0.05);For,patients with type I,II,III in experimental group had lower average FDV than the control group(P<0.01),but patients with type IV have no significant difference in terms of average FDV(P>0.05);The average of MTV of two groups UPFS patients with four types(I,II,III,IV)had no statistical significant difference(P > 0.05).4,In the comparison of atony defecation reflex correlation,Experimental group positive correlation significantly reduced than the control group,negative correlation significantly higher(P<0.05)than the control group.5,In the comparison of rectal compliance of the two groups of patients,Average RC in patients with type I,II of experimental group is higher than that of control group(P <0.05);Type III and IV of average RC after enema differences between the two groups have no statistical significant difference(P>0.05).6,In the comparison of comfort index,there was no statistically significant difference(P>0.05).7,Totally,13 cases of complications(11.11%);3 cases of heart palpitations(2.56%,2 cases in experimental group and1 case in control group);7 cases of abdominal pain(5.98%,3 cases in the experimental group and 4 cases in control group);0 cases of bloody;3 cases of mucosal injury(2.56%,2 cases in experimental group and1 case in control group).The complications between the two groups have no significance difference(P>0.05).8,there was no statistically significant difference in terms of safety evaluation(P>0.05).Conclusion:for the UPFS patients,high temperature water enema has more advantage in improving pelvic floor relaxation loss than traditional conventional enema.Besides,the former one is advantageous to reverse of the pressure gradient,reduced the defecation threshold,reduce conflict contraction,and improve the function of defecation in patients with constipation.
