Study Of Reproductive Health Expendture Accounting In Sichuan Province Based On SHA2011

Objcective :Through accounting and analyzing the reproductive health expenditure of population in sichuan province,to get the information about reproductive health expenditure by quantity,financing scheme,health provider,health function,beneficiary groups,and to get the information about expenditure structures in different types of hospitals,different levels of hospitals,public and private hospitals,public hospitals and maternal and child health care hospitals so as to find financing problems and to provide valuable advices to meet demands of reproductive services of people,to prevent reproductive health diseases and to improve the reproductive health status of people.Method: Based on SHA2011,and according to specific features of reproductive health care expenditure accounting,it collected relevant data by combining questionnaires and insider interviews,and used the Stata software and SPSS software to account and analyze.Result: Recently,the status of maternal and child health has improved a lot in sichuan,but the morbidity of AIDS,syphilis,hepatitis B and other sexually transmitted diseases was still in the forefront of China.The prevalence rate of breast cancer,cervical cancer and ovarian cancer was quiet high.The reproductive health problems could not be ignored.Accounting to the result,the reproductive health expenditure amounted to 24.01 billion yuan in 2014,amounting for 12.95% of current health expenditure,and amounting for 0.84% of GDP.Curative care expenditure and preventive care expenditure were 13.65 billion yuan and 3.25 billion yuan,accounting for 56.83% and 13.52%,respectively.Household OOP accounted for 36.40%,which was higher than that percentage of total health expenditure(31.74%).Most of reproductive health expenditure occurred in hospitals,the proportion of expenditure in primary health care institutions was a little bit small.As for the allocation of reproductive health care expenditure among beneficiary groups,most of reproductive health care expenditure were happened in agegroup from 20 years old to 50 years old.accounted for 78.12%.The proportion of reproductive health expenditure in female was higher than male,amounting for 83.87% and16.13%,respectively.The reproductive health expenditure mainly distributed in urogenital system,reproductive health and cancer.Through analyzing,it can be found:among different types of hospitals,the proportion of curative care expenditure in general hospitals was the largest than that in other types of hospitals,accounting for 71.71%.Secondly,With the administrative levels of hospitals increasing,the proportion of household OOP was much higher.The proportion of household OOP in provincial hospitals was much higher than that in municipal hospitals,as well as county level hospitals.On the contrary,The lower the administrative level was,the larger the proportion of public financing scheme was.Thirdly,between public hospitals and private hospitals,public hospitals had the dominant role,while private hospitals were in marginal position.The proportion of household OOP in private hospitals was much higher than that in public hospitals,accounting for 73.51% and 44.89%,respectively.Fourthly,The proportion of household OOP in maternal and child health care hospitals was much higher than that in public hospitals,accounting for 86.05%.Conclusion:The total amount of reproductive health care expenditure was quite large.The household OOP of reproductive health care expenditure was relatively high in sichuan,especially in the outpatient health care,the population under 20 years old,urinary and reproductive diseases,programs in screening and early diagnosis of diseases.At the same time.It attached more importance to curative care than preventive care in recent years.So the distribution of health function in reproductive health care expenditure should be opitimized.Besides.The allocation of reproductive health care expenditure among health providers was not reasonable,and primary health care institutions should play a role of gatekeeper in reproductive health care.The proportion of voluntary financing scheme was relatively high when providing preventive care.So the problem that institutions used the profits in providing curative care to make up for the losses in providing preventive care was extremely serious.Secondly,the amount of reproductive health care in private hospitals were much smaller than that in public hospitals,and private hospitals did not fully providing the specialize severices.In addition.The proportion of household OOP in private hospitals was much higher than that in public hospitals.Several advices went as follows.Improve the health care insurance of reproductive health care,and reduce the economic burden of individuals.And it was time to improve the mechanism of government investment,to insure efficient supply of reproductive health care.Strengthen diseases control on key reproductive diseases,and improve the reproductive health of population.Built up the ability of providing reproductive health care,and guide the flow of patients.Besides.Private hospitals should identify the role of providing health care and provide specialize severices.
